      During visit from 11.17.21-11.19.21, Amendment to PHI Request (1.17.22) and Statement of Disagreement (4.27.22) I presented evidence of cancerous mass reports to E.R. doctor for cancer evaluation (diagnosis, staging, prognosis, treatment recommendations) &/or to inform medical professionals that I had no cancer treatment to date. On behalf of a third-party payor for payment consideration of $628.63 bill, I request the following information:1.A copy of cancer surgical reports (Late Latent Syphilis or 3rd Stage of cancer, lung mass, adrenal masses, heterogeneous breast masses, anterior paramediastinal mass, pelvis phlebolith), surgical reports for lesions (vagina, splenic, neoplasm in colon, significant abdomen calcifications). 2.All items on an itemized bill must have CPT codes and ICD10 codes to be paid by ********* Please provide a copy of all lab orders with ICD10 codes from Emergency Room and in-patient basis. 3.All notes (1992, 1998, 2006) expired after 10 years per record retention laws (210 ILCS 85/6.17). If you have a copy of notes prior to expiration, please forward a copy of compliance notes (pre-operative labs, ekg, cxr) that were sent directly from providers that is pre-requiste for notes to be used in diagnosis, assessment, plan &/or treatment.

      The police brought a men into the er for a mental evaluation and the hospital just had the office in a room with gentleman suffering from bi-polar, schizophrenia without any help for over 3hours. An police officer should not have to wait with a patient that long. No security or nothing to watch the guy with mental health issues. The paper work was filled out and given to the nurse in the first 10 minutes.

      01/29/2022 I came here through ambulance at 9:30 am.. I was throwing up blood and felt very nauseous. The doctor that collected blood didnt know what she was doing clearly. The doctors here act very unprofessional. There language is very unacceptable. The staff here are very careless. They curse all through the halls. Ive never experienced this at UOC OR NW Hospital. You ask questions when you will be seen, they dont care to respond. Or they have a nasty attitude while doing it. There are no beds available yet they still keep ones in waiting room. They allow homeless to sit in the waiting room and sleep and ****. The waiting room smells like nothing but ****. Like they dont clean! My first time here. This is the probably one of the worst hospitals Ive ever been too. If you have to go here, youre better off dying at home. Because these people here dont care. They even act like they dont care. I overheard a nurse saying she dont give a f*ck. When their to job is to attend to us patients in need or pain with CARE. Its like they dont care because of the neighborhood. I wish someone from BBB can actually be a secret patient or something and just witness how these people are. I wouldnt suggest someone to go here even if they are about to die. This is the worst. Bottom of the barrel. Close this place already. Or start over with NEW ************!

      Please be advised that I would like to file a complaint on the following because I have not received the following **** records upon request from St. Bernard's Hospital.Under HIPPA law for Individual Right to Access Medical Records - 45CFR ******* (a)(1) that includes all billing records for possibility of medical deductions. I ask for a copy of the following:1.UB04 form from 11.17.21-11.19.21 and UB04 for 11.24.21 2.All ICD10 codes for each item on the attached itemized bills including: Radiology, all labs, ER visits, etc.Per ********* all claims submitted from hospital bills must contain CPD coding and ICD10 codes in order to receive payment. Once these records are available, my daughter can pick up the records by calling Ph*************.
