I had Lasik eye surgery about 4-5 years ago which came with a "lifetime warranty" provided I had an eye exam every year. I have complied with this and have had annual eye exams. At my last eye exam I was told my nearsighted vision was degrading (this is what I had the Lasik to correct) so I filled out a form they required had my eye doctor certify the information and My Eye Dr faxed the information to LVI back in November for review. I was told the review could take 60 days. I have called 3 times to get an evaluation set up with LVI to get my vision corrected and they have failed to return my calls. I was told someone would call me back to set up an evaluation and have yet to hear from anyone. I believe my claim was sent to them on November 7th. I am requesting an evaluation to determine if I can have the corrective Monovision surgery that I should be eligible for under warranty.Initial Complaint

ON 06/23/2012 I HAD LASIK EYE SURGERY ON BOTH RIGHT AND LEFT EYE WITH A LIFETIME PLAN AT THE *************, ** FACILITY. THIS PLAN ALLOWS CORRECTION ON ANY VISION DEVIATIONS. AS OF THE PAST TWO YEARS, I'VE HAD A DEVIATION IN MY LEFT THAT NEEDS CORRECTION. I CURRENTLY HAVE TO USE EYE CORRECTION (GLASSES / CONTACTS). AT SOME POINT WITHIN THE PAST FEW YEARS THE FACILITY AT *************, ** HAD CLOSED. WITHIN THIS PAST YEAR I HAVE CONTACTED CORPORATE OFFICES AND THEY HAVE INSTRUCTED ME THAT SINCE THE FACILITY IN *************, ** HAS CLOSED; MY LIFETIME CORRECTION IS NO LONGER VALID. I FEEL THIS IS UNJUST, AS A CUSTOMER, IT IS NOT MY FAULT THAT ONE OF THE LOCATIONS HAS CLOSED. MY LIFETIME CORRECTION SHOULD STILL BE HONORED, OR I SHOULD BE REIMBURSED FOR THE SURGERY. **************** IN RESOLVING THIS ISSUE.
