      *********** on ****************, Suite A is trying to charge me for ******19 tests myself and my son, ******************* took around May 2022. At this time, the ******19 tests were advertised as FREE with OR without insurance. We recalled this being advertised at the time of the tests and was also able to use the website WayBack Time Machine to confirm. This website archives other websites and captures the contents of websites at a point in time. We did not have insurance at the time due to a change in employment but they billed our insurance company anyways. At the time, we told them the situation and they reassured us it didn't matter because the tests are free with or without insurance. This was when the tests were done in the garage parking lot. I didn't realize aayu sent a bill to my old medical insurance company anyways at the time. **** has since received a refund request from my old insurance company and now they are trying to bill us $280 per test due to my old insurance company requesting the refund. We believe this billing is fraudulent and incorrect because the Covid-19 tests were advertised as free regardless if we had insurance or not. We've tried addressing this matter with aayu clinic by phone and have sent multiple emails to aayu clinic at ***************** and *********************************** that have gone unanswered. We are unsure what to do at this point. Is there anything your office can do to help us get a follow up from *********** or agency you can refer us to so we can file a complaint against the clinic for fraudulent billing? I'm worried they will try to send this to collections and ruin my credit and livelihood for a bill we should've never been charged with in the first place. Any assistance you can provide is greatly appreciated.

      I have been asking Aayu Clinics-Lakeview for an explanation of charges since December 2022 via email as their website requests, with follow up emails January and February 2023, all to no answer. My bill through the patient portal showed erroneous and inconsistent charges for the same levels of service and same CPT codes for various visits. Part of this request is that they are unnecessarily requiring me to have telehealth visits to refill a prescription, which is legally not required. These medication refills do not require a provider visit for each refill. Finally getting ahold of them, the ****************** has not provided "reasonable explanation" of charges as is required by law (410 ILCS 50/3) Medical Patient Rights Act Chapter 111 1/2, paragraph ****, Section 3 (b). Aayu Clinics-Lakeview claims my insurance charges $50 per visit, although my copay is $30. I have no evidence or record of my insurance company attempting to charge/collect $50 for these visits. Thus, I believe this to be erroneous charges. The billing department wiped all outstanding charges without further explanation of the charges as I had requested. The practice and billing department has not fulfilled my requests for records of the fee schedules used to attempt to charge me, and still have not provided reasonable explanation as to why I had inconsistent charges for the same level of care and visits. As of today 5/1/23, the clinic still will not send me copies of forms I have signed since August 2022, nor reasonable explanation of fees attempted to be charged to my account. The ****************** and ******************* have not appropriately, timely, nor reasonably responded and completed my requests per my right and what is just.

      Hello,Last year I visited an Immediate *********** to receive treatment for food poisoning. They collected samples, ran tests on 9/22/2022, gave me the results, and I paid my bill with them directly. The name was *********** in Wicker Park, *******. I received my results over email and no longer had any communication from them. Fast forwarding to now, April of 2023 I just got a call from a *********************** (***************************) that I have an outstanding debt from Northwestern Medicine Healthlab for Lab testing? I never visited or received treatment at any ********************* at all last year, so I had no reason to believe any billing statements from them were legitimate. The billing statements were sent to an incorrect address, so I never had a chance to address the issue. There was no attempt made to contact me through mail at my correct address or through email. This bill needs to be recalled from collections so I can pay it and any marks on my credit history should be removed. Thank you for your help!Best regards,**********************
