      The dental clinic owes me $121. They charged my family for services that were already paid by my dental insurance. The clinic agrees that they owe me this money. For more than 3 months now, every time I call, they have been giving me vague answers. I’m still waiting for the reimbursement. Last time I called Maple shade in Dunlap on March 20th, they said that they had processed it and sent to their head office in Florida on February 28 and should’ve received the check for $121 by now. I even got their Florida office number, called and left a message to call me back to resolve this issue on March 28. No one has called back.

      I went in to this dental office on April 4, 2022 to check on a loose crown. The doctor tried to yank it out and said nothing further could be done. It took 5 minutes and I received a bill for $142. I called and gave them my insurance information on June 7, 2022 after they failed to add it to my account at the time of service. I received another bill on June 30th and called to give them my information yet again on July 1. I received a past due notice on 7/17/2022 and proceeded to call them every day to see if they have submitted this bill to my insurance. They have not. On 7/20 I was told the office manager was waiting on someone else to submit the claim and it would be submitted the following day. I gave them a week to get it done and called back today 7/27/22. They have still not submitted the claim. I asked to speak to the manager. I have asked before as well with no luck. ******* told me I couldn’t speak to her at the moment but her name is ****** and I could ask for her next time. I don’t want to call again. This is getting ridiculous. Why won’t they submit my claim? I’m not paying this bill.

      I had a visit to the dentist in June of 2021. In January of 2022, I received a bill from them. This was the first bill I had received from them. It claimed that I owed them $43.63 and I paid it immediately. However, a week later, I received a notice from a debt collection agency, claiming that this bill was well past due. I have responded to the debt collection agency that the bill has been paid. Upon calling MapleShade Dental, they claimed that I had been billed in September and October of 2021. I have signed up to receive e-mails from the post office with image scans of my mail and I confirmed that a bill from MapleShade Dental was never sent to me. They also claimed that billing was handled through their billing department in Florida and they had NO contact information for this billing department. My complaint here is as follows: MapleShade Dental failed to send me a bill for services received in June of 2021 until January of 2022. They then claimed, erroneously, that I failed to pay them and sent my account to a debt collector over $43. If you look at the 2 files I have uploaded, the statement says I have until January 15, 2022 and then the debt collections says my debt is as of 12/31/2021.
