  My mother (*************************) had physical therapy through AF Therapy Clinic LLC, starting in November 12,2022 through January 14, 2023. My mother lives in a memory care facility and has dementia/alzheimers. She was never charged her copay and was not billed for the copays all bills were sent to her insurance United Healthcare. I handle her business affairs and I was not told that she needed to pay her copay at any time while she was receiving therapy. I have never heard of AF Therapy Clinic and received no bills until July 31, 2023. My mother was sent a bill for over ***** dollars...after calling AF Therapy Clinic LLC, I was told those are copays. These copays were supposed to have been collected on the days of service...she never received a bill for the days of service nor did I. My mother is on a fixed income and has dementia and I am in charge of her payment of bills. I am completely thrown by this unexpected bill that is a compilation of 3 months (November-January) that is just now being sent (July 31st) with an expectation of payment.

  Charges are legitimate and management has been playing phone tag with the consumer.

  I have asked for documentation of patient care on everyday that my mother ************************* was treated. I do not understand the charges and the fact that she was seen 84 times in a 3 month period is to me excessive. I need documentation of treatment for the 84 times she received care.

  I rec'd a bill in the mail on 6/30/23 for $604.54 for dates from July-Dec of 2022. I called to see what these services were for and the man who answered the phone could not pull me up in their billing system and said the company on this bill he had never heard of. I googled the name of the business and nothing appeared. I think this is a scam and I want to make sure this company does not try to take money from anyone else. See attached bill I rec'd and please make sure they do not contact me or anyone else since not a real business.
