      I went to LA *************** ER on September 3, 2021. I paid my $100 ** pay(for ER visits) for my *************** Shield HMO Insurance, I have had HMO for over 25 years and never had an issue. Almost 1 year later I get a bill from this **mpany for $1,671.00. I called my insurance **mpany and emailed them the bill and they said everything was all good. A few months later I get a letter stating I'm in **llections for the amount due of $1,671.00. My credit s**re went down 100 points!! I was told by the **llection agency that the doctor was not in my network and that's why I was sent this bill. How is this my problem. I paid my ** pay and gave the hospital all my insurance info. This should not be an issue!

      This company has sent a bill for $1,686.00 for an emergency department visit on June 26, 2022. The only statement details include this date and a provider, "****** MD". While I did have an emergency room visit on that date, all charges were covered by ******** and private insurance.The bill includes another company address:PO Box **** ******, ** 43607-0475

      A bill from Illinois Emergency Med specialist LLC was sent to a debt collector . I tried to contact them with the number I found here but the line just keep on ringing and will disconnect after . We have medical insurance that can cover the bill . I cannot get hold of this company .

      I am under medical care for the treatment of several health conditions. I was prescribed an opioid medication, nerve medication, and muscle relaxers post spinal fusion surgery on January 26, 2023. I was given medication from back surgeon before 6 week follow up; then prescribed a lower dose from pain doctor in March. I had to have dental tooth extractions and my dentist prescribed tylenol 3. I took the 10 over 2 days but still had pain. My dentist stated that he could not refill so I used the medication I already had. I had a refill prescription sent to the pharmacy but the pick up date/time was delayed because it was too early. I mistakenly decided to self medicate with tylenol extra strength and pm. I used these over the counter primarily for insomnia treatment. I ended up taken more than label suggested and started having what I thought was overdose symptoms. I sent a message to both my pain specialist doctor and primary care doctor. I was told to go to the nearest ER immediately. I arrived at 3pm CST and was discharged at 8pm CST and told I didn't have high levels of tylenol so I should go home and take more tylenol to treat pain. When I asked why that was the recommendation because the problem lied in me not getting any effects from normal dose and didn't know how more tylenol would help my pain. I was told in the nurses station area of the hospital that I was drug seeking and should be arrested. At this point my discharge paperwork was at the printer. I asked for my paperwork but while waiting the doctor. DR ****************************** kept insisting I was a drug addiction and should be arrested. I asked why he was saying these things too me when I have documented conditions that are treated with controlled medications. In front of all staff, other security, ******************** said he looked up my prescription history and I was seeking drugs and I need to be put in jail. I asked him why he would say these things to me when I came in for taking too many tylenols and my doctor told me to come get evaluated.

      On 12/12/21, I - ***********************- received care from a party who Illinois Emerg Med Specialists LLC /physicianbilipay.com bills for (owned by the company I am filing this complaint against). On several occasions I have provided representatives with my insurance information, as well as have made numerous phone calls to ********** Blue Shield of Illinois spending hours with them trying to get this company to stop sending me bills. It is the incompetence and or unwillingness to properly file insurance claims or potential fraud in order to collect more money from me than the insurance negotiated contract amount they would get from my insurance group. Whatever their issue is, they are refusing to collect from my covered insurance provider and have been harassing me demanding over $1600 from me for a year. I am tired of their games. I want them to properly bill my medical insurance or recover their bill from my auto insurance which included medical coverage. They should not continue to harass me after I have repeatedly provided them with all the information they need in order to properly claim from insurance. Stop sending me bills. Stop threatening me with collections. I will get both law enforcement and my attorney involved if this continues.Desired Settlement:Stop contacting me. Cancel my liability and collect from my insurances!

The accounts in question are listed below. Please send me copies of the documents that you have in your files as of this date that you used to verify the accuracy of the accounts listed below.
