      I emailed the Center for Covid control after they were shut down to ask what would happen with my personal information, my passport, SSN, and insurance card and they never responded to my email and now their website has gone offline.

      On January 13th 2022 I went for COVID testing at this business located at **************************************************************. They required my drivers license number and picture of my drivers license along with my contact information and medical insurance card. I was told I would receive test results in 3 hours. I never received any test result (antigen or PCR test) and had no ability to contact them. The workers also did not handle my test is a manner consistent with a true lab test, which made me skeptical/fearful after the fact that they were not a legitimate testing site. I fear this place is a scam that stole my identifiable information without running my tests.

      Company sold my health information. I am getting text about having type 2 diabetes and magical cures. Do not trust this company. I also question there sanitary practices and test results.

      On December 28, 2021, I went to the east side *******, ** facility of Center of Covid Control and took a rapid test. The only issue I have with this situation, is that they "required" a photo of my driver's license when registering online and my license number on a small sheet of paper I included with my test. I'm sure there were many other people who provided this information as well.Thank you

      I was very Ill and couldnt get a test anywhere else. We were in line for 8 hours. 3 of us were tested. I have all of my information and never received any results

      I went to get a COVID test at their *********, ** location on 11/02/21. I was asked for a photo of my state ID and of my insurance card through an online form before we started. I received my rapid results via e-mail quickly stating that I was negative and was told I'd get my *** results in ***** hours unless I paid $100. If that fee was paid, I'd get the *** results in 24hrs guaranteed. I received the results 5 days later, but they got lost in my inbox. I followed up stating that I hadn't received my results yet and got a quick response from a WIX email address stating, "Unfortunately we cannot find you in the system. This may have been due to an error in transportation." The whole organization is sketchy and needs to be stopped.

      Got my child a Covid test and they asked for a picture of either passports and photo ID. I didnt think of questioning it at the time, but now have learned of fraudulent activity there

      This company asks for too much personal information. Also their locations seem very iffy. If I had went into the building before entering all my personal information online I probably would have never continued the process but since the ** code they gave me outside the door requested all my information and it was already in their system I went inside and got tested. No wonder why they allow a parents ID To go under a childs information, because they cannot steal a childs identity as easily as an adult.

      12/23/2021 was the date of the Covid test collection for myself and two members of my family. We tested at the Hiawatha location in ***********, *********. The results were promised the same day, but I actually received all 3 results by email on 01/06/2022 weeks later. The documents I received with the results state that the collection was completed on 01/06/22 which is not true. Additionally, there was only one person working at the site doing collection, and she could not answer any questions brought forth by people waiting in line for tests. Numerous people (including me) asked her what kind of test we were receiving and she said she did not know. People excused this because she was clearly being overworked, with not one other support staff. As we were giving our samples, we noticed that test samples taken from people before us were all still sitting behind the booth even though she had already verbally announced their results to the lobby. We waited for almost 2 hours in line and not one person received a positive test result (results were given out loud, less than 3 minutes after collection, with peoples names by the worker for everyone to hear). Despite being told I had a negative test result, I tested positive a day or two later at an urgent care after developing serious symptoms. My entire family developed symptoms after being told our test results were negative. I feel like we were scammed out of personal information (IDs, insurance information, home addresses, phone numbers, etc) while attempting to do the responsible thing and get tested before returning to work/school. In the lobby of the testing site, there were 10+ people crowded together, some coughing and some with masks worn improperly, waiting for the single staff member to give their results out loud because she said that the email results may take a while to get to us. I genuinely believe this is a large-scale scam. Their website stated that they were affiliated with a CDC approved lab

      On January 7th 2022 I went to free COVID testing site at Park St and W Washington in ******* **. They made me take a picture of my drivers license and insurance card to register. I got negative results emailed less than a half hour later. Now I hear these sites are fake and I am very concerned about what they will do with my personal information. I want them to destroy any record of the info I got scammed out of; I should have known this info is not required for a COVID19 test but I fell for it. Also there was one person in the whole place registering people, giving them a swab to self test, and collecting swabs. They seemed quite careless with juggling so many people and tests and barely keeping track of things.
