      I'm filing this claim for Theft of service. On 4/26/2023 I went to Affordable Dental in Shorewood IL and paid a total of $12,341.00 in full. I went in to get 2 teeth removed, but the doctor ********************* convinced me into getting implants and she removed all of my upper teeth for implants and they're still in good condition to this day. In the middle of surgery, she stopped and sent me to a different dentist located in Aurora, ** with doctor ******************* to finish the process. He wasn't willing to do Xray's, or the lower bottom procedure. Nor did he not want to reinsert one of my upper implants that came out. He stated in frustration that " All they sent over here was you and no Treatment plan or no money." I still have infection and swelling as i write this claim. I had to get pain meds, antibiotics and mouth rinse from my PCP. I went for an appointment and both locations were temporary closed. After communicating with them via phone and not getting any results. I drove pass the Shorewood location 6/27/2023 and noticed it had reopened. I went there in good spirits and was informed that they had sent me out a check that I never received. so, i asked when it was sent, and the amount was. she told to have a seat while they look it up. and they called the police on me they informed me I wasn't the only one and i went to get an incident report 6/29/2023. the next morning, I received a Next Day Express Air via *** from Affordable Dentures with a check dated from 3/03/2023 with a void after 90 days so my bank refused to cash it. they also sent a letter dated 5/22/2023 in the package with the check saying I had 30 days to request my medical records. I received the package on 6/26/2023. I sent 3 certified letters to get my medical records and no response still to this day. They still have both upper/lower teeth. i miss smiling and communicating with people and being able to eat. PLEASE HELP!!!

      The Following patient was provided a  dismissal letter via certified mail on 5/22/23. Along with the letter there was a refund check  for any non rendered service the patient prepaid for. The patient was also  given a referral to the ***************************** for dental care. Please  also see provided is a bank statement with the cash refund check on July 3rd 2023 by Mr. *********. . Further  Affordable Care has mailed a  copy of patient records to the above provided address of patient to the patient on 8-- ****** and Waived our record fee for duplication of files. Should there be an issue with reception of these records please have client reach out to our patient service team at ************** 

      We wish the client all the best in future treatment.

      I am rejecting this response because: I never received a dismissal letter and they never sent me a referral to see anybody else. i didn't recieve anything from them until the end of june when i made the police report and then i got a *** package from them afterwards. i didn't get my teeth i paid for it. the credit union only covered the check due to me having money in the account to cover it, but they gave me a problem due to check being over 90days when they sent it out at the end of june. i have a time table of everything that happened and the *** letter they sent was time stamped so it wasn't 5/22/23 when they sent it. i had to pay to go see another dentist to try and fix what they messed up and i still have no teeth in my mouth to this day. i want to be made whole again.
