See attached letter: Joined Profile by Stanford on January 29, 2021 Invoice No: XXXXXXXXXX Cost: $1079.00 (Annual Membership) + $200.00 No services rendered after January 29, 2021 In severe back pain with diagnostic testing done. No services were rendered from Profile by Stanford March 22, 2021 - Had back surgery - *********** of L3 & L4. No services were rendered during this time and no contact with Profile by Stanford. July 13, 2021 - Right total knee surgery Experienced Hypertension following COVID vaccine and surgery so dieting was not recommended. Notified Profile by Stanford. Apparently, they do not understand the concept that dieting is not recommended during critical health issues. As a nurse it is concerning that my health issues were not important to these people. August 27, 2021 - Cardiovascular tests were done to rule out heart problems. Called Profile by Stanford in September and November to discuss my health issues without success. The options that were provided was another diet plan. Their recommendation: Pay another $399.00 for another year. No services were rendered during 2021.Business response

Business Response Contact Name and Title: ***** ****** - Area Manager Contact Phone: XXXXXXXXXX Contact Email: ************@profileplan.com ***** joined our program and signed Profile's membership contact on 1.29.21. Our contract states "To terminate, a completed membership cancellation form must be presented to the store within 3 business days of your membership effective date". Unfortunately, ***** did not request a cancellation until 7 months into her contract. Due to some of *****'s medical considerations, we did adjust her program to try to accommodate her special needs, however, ***** chose not to participate in the coaching from there on after. If ***** would like to come back, we would be happy to let her pick up where she left off.
