      I went to Midwest Sport and Spine for chiropractic treatment and they started treatment - in the meanwhile I passed out due to COVID shot and exceeded my limit. Therefore I asked them to apply full payment from insurance and share details with me and issue me the credit for extra I paid. They did not do that for 15 plus months and no answers to multiple attempts to receive statement updates. Last visit was in August sometimes in 2021 and I received a bill in December (late) 2022 saying I owe them money without details. I asked for details and also asked Dr to call me with explanation. Didnt receive that information at all. I just receive a letter from collection for them. This is at best mal practice of business. They have not been able to provide me details of charges and credits to date but instead sending a different number to collection - attempting to collect something I dont owe. Please help with pushing this business get on the right path or shutting it down as they dont deserve to survive with this attitude

      I was given a proposed treatment plan with pricing laid out. I specifically asked if this was a contract, as I could not commit to any length of time with my work travel. I was told no contract, just let us know when you can't come anymore and we will settle up based on how many appts you have had. I went there for 3 months, and I started to see charges on my EOB that my insurance was not paying for. I told them I did not want those services if they were not covered, and I was told not to worry, because they were removed in my discounts notated on the pricing sheet I was given. I paid them over $1000 out of pocket and my insurance company paid them several thousands as well. When I cancelled services because they were adding treatment to the **** that I had not received, I was told they were going to go back through all of my past appts and remove any discounts, because I was not agreeing to keep coming. This removal of discounts created a **** for $1300 that they are now trying to collect. I have dates that I notated I had to leave right after my adjustment, and could not stay for exercises or light therapy, and they still charged me for these services, which is why I quit going. I have attached a copy of the treatment plan I was given that clearly shows what they agreed to write off as part of the plan. There is no place in writing that they state you will be up charged and discounts removed if you don't keep coming for more appts. I am certain charging for services I never received is illegal. I will not pay this ****, and I will get an attorney to take them to court, if this is not resolved.

      This patient was seen in our office and provided a quote to estimate financial responsibility. It is not a contract only an estimate of charges. She came to our office 16 times. You will see the statement reflects services performed. We didn't charge for any services that were not performed. As you can see her insurance paid but the remainder is just her co-insurance and co-pay. She had set up a monthly payment that based on her estimate, spread out over 6 months to help with the financial responsibility. She only paid two of those payments. The remainder is still due. We sent her to collections which is why she is filing this complaint. Patient also tried to lie to the ***************** to say that she was in another state at the time of service. We do have a time stamp to show proof that she was in our office as well. She is headed to small claims court now as we have exhausted our attempts to collect this from her.

      I have reviewed the response made by the business in reference to complaint ID ********, and have determined the response would not resolve my complaint.  For your reference, details of the offer I reviewed appear below.

      The payments that were set up to spread over 6 months were supposed to be for 36 visits as noted on the attachment. If it was not a contract, then I should not be charged for visits I never had. There were several visits I did not stay for exercises and laser and only received an adjustment, however all services were billed for each appt on your sheet. I will not pay this. My next step will be to file a complaint with the *********************** and the Attorney General.
