      I was going to this lifestyle clinic to better my life as a man, I was getting testosterone shots as well as other shots I did from home and taking pills with other supplements. I had been asking for month starting in July to speak with the doctor about having issues with the medication and shots. I was having horrible headaches and stomach issues while taking these shots along with one more medical issue I'd like to not disclose. I had repeatedly been asking to speak to the doctor which is suppose to be the process if I ask and yet the female assistants or nurses did not set up to happen. They have it on the charts of my issues and then I talked to billing with other people within the business for me to stop my contract and payment and was told to try billing and wait so I did so. I had called numerous of times and even sent some emails to them no response. Finally I called over and over showed up in person and also spoke with a few people. After all that 2 months later I called again and the same women who told me to contact all the other people was able to assist me at this time which was odd since she could have before. I told her my situation and also that I'd be moving in which I did not move and that it was all about how it makes me feel so I can not keep doing this treatment she finally in September stopped payment and would not refund for previous months even though I had not been going in and had not been receiving my medication via ******** services from Florida as before. I had paid a total of $432 for two months and did not receive any refund any medicine for which I paid for it services. The women was highly rude and the doctor avoided trying to contact me by phone when I had been asking for months. I know the medicine was the issue due to I did stop taking it for a week and the headaches and stomach issues went away then I started back on it and the same results again, (headaches and stomach issues). I would like a full refund please.

      Business Response /* (1000, 5, 2022/01/18) */ Hello. We are a membership based medical doctor's office. ***** ****** signed a one year contract with us on 06/02/21. Mr. ****** signed up for Dr. ******* to be his primary care provider and wanted to see him for all medical related issues. A few days later he called and wanted a refund saying he changed him mind, he only wanted to be seen for Hormone Replacement. He wanted to drop his contract from 250.00 to 216.00. We typically don't do that but we made an exception for him. He was supposed to pay $216.00 for one year until 06/2/22. He did receive injections of testosterone in June and July. In July the patient stated that he was moving out of state and would like to cancel his membership. We explained that we can and do service out of town patients but if he would send us a copy of his new lease, we would cancel him. Even though that is not our policy. We uphold a one year contract and patients are told that and sign that they understand that at the day of signing the contract. He would call constantly and get hostile with us because he couldn't get refunded for July and August even though he never sent us his new lease. We have documents that he called in about his bill but nothing about the medications making him sick until after he couldn't get a refund. He could have made an appt with our doctor or any other provider at any time for sickness. All he ever called us for was "moving" which did not even happen. We do have notes of him calling in about his bill, none of which were about sickness until he was told that he could not cancel. Even then, we went ahead and cancelled him just because he kept calling daily wanting refunds even though he used our services during that time. The last charge, he disputed so we didn't even get it. We only collected 3 payments of 12. We did stop the billing in Sept and he has not contacted us since then, nor have we contacted him. Thank you for your time. I did not at any point be hostile with anyone in the office. I also did try to get an appointment with the doctor numerous of times and yet they did not call me back or set one up. I am obviously a grown up and can figure this all out for myself in which I did. They should have plenty of documentation of me saying it is making me sick I have witnesses on my side from phone calls and I did call slot to only be told to contact someone else in which my phone records will show that. This is illegal for a company to not write down patients info when they are saying what is going on and I 110% told a young women ******* there at the beginning of July it was making me sick. I'm fact I told a few other women in there as well it was and ask if I can speak to the doctor they said he would call me which he did not. A women there said he tried calling me but my phone record's do not lie. This place is a joke, and also was not told they had out patient programs either in which Arizona does not have them there. That is where my aunt lives at and I was going to move and decided against it. That had nothing to do with their program at all what the issue was in the beginning of July I was complaining to a few women of my concerns with how it was making me feel sick. Testosterone shots are once a week in which I fully stopped after the first week. Either they are fabricating their charts but I do have a witness who will testify on my behalf. I had them on my car speakers via Bluetooth with my other party listening for my proof of trying to cancel. Yes this was a year contract and also that was false about going down in a plan hence anyone can do it if they do not want him as a doctor or a cheaper price. That was not some exception they made at all. ******** was who I signed up with and she told me at anytime I wanted to downgrade my plan I could do so. I was fair with everyone I did not yell or at any time be hostile I did what was ask and I did also take a personal copy of the lease which their camera system will revile that, the lady at the front desk took my copies made their own and handed them back in I am sure they will have that date on recording. I did get hostile in a slight way at the end when the women I was speaking with knew of the medicine which was making me sick and also told me to go through loops and hoops for months when I was calling trying to get an answer. I also tried calling to speak with the doctor or to come talk to him in person they did not let that happen. I'm which now this is a complete dishonest reply from then stating false statements. The same women who I talked to finally said bro by the lease in and they will cancel me out but I had been trying for months. Also before hand I was not advised to bring a copy of the lease in until I was ask in which they knew that and told me. To much back and forth with this company and I do not need to be dishonest about this. Either I have a refund or by law with what I paid for the medicine to be sent to my house for the $432 in which I did pay for. This is not some free get money service from me if I owe anyone a product for which I paid for by law I will have to give it to them or refund them. So either way their choice I would like the products I paid for and or the refund. I have complied with Marshall lifestyle medicine and did what was ask in their charts including *******s chart I did stat the facts about how I was feeling. This is by law a requirement to document and not just let it go by. If I could have taken the medicine I would have and we wouldn't be here now. It is not my fault my body did not adjust to this medication or treatment and I really wish I could have stayed with the program by far yes because some of the people are great and the doctor is but I could not keep on taking this medication and I ask if I have problems would that alone let me opt out of their contract and ******** said it's possible and if that time comes we will speak with the doctor in which I tried too. This is not like I wanted money back because I was not satisfied it was due to me actually having symptoms from the types of medicine I was taking. I know what a contract is I know how they work. I apologize for the inconvenience again but I believe a full refund is what is owed due to I did all the proper steps they ask and at the end after wasting more time for us both they lied and did not try to help me as a patient. I have issues with a lot of medication and have allergies which they know to a lot of antibiotics and ask would their medication possibly have an effect on me I made sure to ask all the questions before jumping into something I couldn't do. ******** reassure ld me with some other women there that I would be ok. I went off their experience in that field. I was even hectic to get a shot but was reassured everything would be ok at first yes it was ok nothing was happening but as a month went by I started to have stomach issues with really bad headaches and as I said before I did my own testing with stop taking the stomach shots to combat with the testosterone and the head aches went away, after starting that shot back which was an important shot they came back and I mentioned this plenty of times. I can not help what my body will take or reject at anytime. It is the medical team's responsibility to listen to the patients and find a solution that benefits me as the patient.
