Initial Date of Transaction: Unknown Company switched accounting systems and does not have a record of the two invoices sent to customer. LSU/WebTPA Explanation of Benefits shows Provider listed as "********* *******" now doing business as 02 Solutions. Amount Paid to Company: $451.14 (Part Personal Check $228.07 (July 18, 2023) and Part Medical Reimbursement Plan $223.07 (May 26, 2023). Numerous Phone Calls Placed to Accounting Dept., 02 Solutions, Attention to *******. Sent Certified Mail (10-25-23) to ******* at 02 Solutions explaining problem of overpayment to 02 Solutions, along with documentation. LSU/WebTPA Explanation of Benefits sent to Customer (***** *******) states "Remaining Patient Responsibility" is $52.47. Customer had overpaid the company based on the two invoices from Louisiana Lagniappe for a total of $541.14. ******* issued a credit to the (debit card) Medical Reimbursement Plan for $221.00. Customer still overpaid 02 Solutions $228.07 and cannot resolve issue with *******. ******* stated the company has no record of the invoices sent as their accounting system has changed. We have been awaiting resolution since October 2023 when certified letter and documentation was sent to 02 Solutions.
