    I was referred to the practice by my insurance UMR United HealthCare Choice Plus; my first visit was a consult for TMJ on 10/25/23. I paid the upfront fee of $342 with the expectation of reimbursement once my insurance claim is processed. We discussed the treatment plan, and the office staff stated she would contact my insurance the following day to start the authorization process. I work in healthcare; so I am proactive with my insurance and also contacted my insurance for benefit information. Because the treatment offered require I have someone with me, I was trying to schedule transportation and the treatment when it was convenient for my job and my transportation's job. Every day I kept getting; I'm the same information from the office that they will contact my insurance. Then the insurance person would be out of the office for a few days, but she had reached out to my insurance and would follow up when she returned. So waited until she returned before contacting the office and my insurance company, my insurance was still stating not information was on file nor was authorization was required for the service. On 11/7/23 a case was finally opened with my insurance by the office; I was informed by an insurance rep that no authorization was required but predetermination was required. The office was need to submitted medical records. I contacted the office to provide the information was given, and I was told that effective 11/7/23 they will no longer accept United HealthCare Choice Plus insurance. This office still has a contractual obligation will United HealthCare Choice Plus but is refusing to provide service to their members. They told me I can pay a cash price of $2000 for medical treatment instead of submitting the requested information to my insurance. I actually reached out to their billing office, because my insurance paid them in full so I should have an overpayment on my account.
