On October 17th , 2023. I received an email stating that I was approved to work with IntelyCare. I was excited due to this being my first agency attempting to work at. I went to upload all required documentation and an error hit regarding my social security number. I emailed them immediately on the mistake. A few years ago my identity was stolen, all of my important details were stolen from me. Ive since then have had to file bankruptcy due to loans and credit cards being put underneath my name, Ive also found out that not only with IntelyCare. This individual has used my identity to work through clipboard, and Shiftmed as well. Clipboard and ******** has worked with me to close the accounts and have given me details on what was said by the person pretending to be me, also giving me the opportunity to work with each company for myself. Each picture used was from different people of different shades of African American, but the address used was from my Drivers license. They believed that I had children, when I do not have any children at all. *** worked as an intern to get me prepared to be a nurse practitioner for the last year, I havent used my nurse aide license for my personal gain. When I asked IntelyCare if they could give me information on the situation and what happened underneath my license they stopped responding and left it as me not being able to be rehired when I have never worked with them. Anyone could have my background check sent to where they need it to go since if it is already on file with the state you could request them to send it directly. I have asked IntelyCare when the last time a drivers license was updated I also did not receive a response for that either. I am just dumbfounded on how still after all these years my life is still being messed with my this individual.
