On March 18, 2023, I was prepped for a crown (tooth #**) by Dr. H38393**63430353735H **** in Woburn Mass, whom I have been seeing for the past five years. Her provider address is **************************************************************, Mass. Tel# ************. This was the only tooth Dr. **** worked on that day and the only tooth that was the entire reason and focus of my visit. Dr. **** finished prep on my tooth #** on March 18, 2023 and took an impression of that area to fit for a new crown. After which, a temporary crown was placed, I went home that day.On April 28, 2023, I received an itemized bill for Crown #** in the mail for services performed on March 18, 2023, minus what BCAZ Dental paid on this claim. When I proceeded to inspect Dr. ***** ********** immediately noticed that Dr. **** was charging me for a second crown for tooth #** for $1,470.00. There was absolutely no work performed on tooth #** whatsoever and a brand new ceramic crown was ABSOLUTELY NOT replaced by Dr. **** for #**. There currently is a perfectly good crown in at #** which is has been there for the past five years. I tried to call Dr. **** the following Monday morning on May 1, 2023. The receptionist answered and I proceeded to explain the situation to the billing receptionist. The billing receptionists answer was that I did have a brand new ceramic crown placed at #** and it shows in our xrays. I went on Dr. ***** website and sent her office an email explaining the billing error and to mail me a new bill, excluding the crown for tooth #**. I did not hear back from Dr. **** after that mail was sent. On May 2, 2023, I called BCAZ H3**532343**937303032H and explained this billing error and new crown NOT placed for #**. They called Dr. **** while I waited on the line. BCAZ H3**532343**937303032H came back on the line and told me that the dentist said there was a hole in my crown so she had to replace the crown. THIS DID NOT HAPPEN AT ALL. Dr. **** and myself did not even discuss crown #** whatsoever.Want a credit of $653.90 on bal.
