Brown, Jr. Funeral Home Name of Deceased: Eric ********** Date of Death: 09/08/22 Conference w/ Funeral Director: 09/12/22 Upon making funeral arrangements every family is advised insurance is an option to use for payment of services rendered. If insurance is chosen as a method of payment, family is given a copy of our INSURANCE GUIDELINES. In these guidelines it is stated "It is the responsibility of the beneficiary to reach out to the insurance company to discuss residual payments if applicable." These said guidelines were said to be understood and were signed off by beneficiary, Elizabeth ********** 09/12/22. ID of Remains: 09/13/22 Cremation Completed: 09/16/22 Received notice of Funding by Insurance Company, 09/20/22 Memorial Service: 09/21/22 Cremated Remains & Death Certificate were picked up by Byron ********** 09/21/22. Beneficiary, Elizabeth ********** signed claim forms 09/21/22. Byron ********** called 09/26/22 for status. He was advised by our Insurance Administrator, Tiffany ******* documents had been sent to our third party funding company C&J Financial for handling. Upon receipt of documents, C&J Financial would have to forward to Insurance Company-One America Life Insurance. Eric ********** called again on 09/28 for status and was advised to please allow mail time. He was also advised entire process can take approximately from 6-to 8 weeks We received notice of Missing Paperwork from our 3rd party- C&J Financial, LLC, Friday, 09/30/22. We then on the same day being 09/30/22 forwarded all necessary paperwork that included an original copy of the death certificate via mail back to C&J Financial. We received notice 10/08/22 from C&J Financial all documents had been receivednamely...
