      My mother received service from a LifeBridge Health care doctor on September 1, 2022, at an assisted living facility. She passed away September 6, 2022. Medicare paid a portion of the billed amount. I paid the remaining balance on my credit card on November 10, 2022 (Confirmation of payment No. *******). Secondary insurance then ended up paying that amount so LifeBridge had a credit of $28.91 Acct ********. LifeBridge sent a refund check, in my mother's name, to the Assisted Living facility on June 5, 2023, nine months after she passed away. I have been calling LifeBridge since August 8, 2023. Since that time I have made over a dozen calls, specifically to Tori *******. She confirmed the refund check was never cashed and would issue a refund check me. I even requested a refund to the credit card used for the initial payment if a check was not possible. To date, I have not received any return calls or the refund. It may be a small amount but this seems to be a negative pattern by this company based on other complaints. I just would like them to refund what is owed. Otherwise it is stealing.

      September 28, 2022 at 2:30p.m. I went to see Dr. * *** for hematology. I paid my copay which was $60($58.50 w/ discount for paying right there) up front as he is a specialist and it is what my plan requests. The date is shows received was 9/30/2022. Then on 10/02/2022 it was processed. I should have zero balance left to pay. Instead I kept getting notices for due payment on my cell phone and in my email. REPEATEDLY. When I've called no less than 4 times trying to get this straight I would hear different things like they could not see that I paid for it. So I did what they asked. I sent the claim that shows I paid it. I sent my bank statement showing they took out and it posted for a lowered amount of $58.50 due to me paying right there on the spot. I've talked to multiple people all stating they will call me back and I never get a call back. I've emailed as they requested to the emails they gave me and no response. Then when I call back they seem to be annoyed by it. I just want the payment to be properly applied to the visit and them to stop harassing me for doing what I should which is paying a bill! This is not how a paying customer should be treated for deciding to use a doctor under their medical wellness company. I would like this resolved and I would like it resolved with proof. I'm not getting warnings of it going to a credit report.

      I paid LifeBridge's 211.77 for a bill prior to my health insurance paying the bill in full. they don't deny owing me the money. but they won't refund it. my health insurance paid this bill on April 8th I have called and written them requesting my refund. Iwas asked to wait 3 wks. I waited a month. Then called back. they took my phone number and promised to call back but never did.

      my ck 993 was sent to pay visit XXXXXXXX. Check clearedb7/6. On 8/3, I receive past due notice and then found the money had been applied to another visit for which I never received an invoice. This is a common practice to apply funds to oldest charge, not necessarily the bill I intended to pay. As part of my Section 8 housing, need to document medical cost as it affects my rent. I need the bill with cancelled check attached showing bill is paid. When you apply check to bill, I have not received, I have a problem. Believed your way of applying funds to oldest is done to make your aging reports look good but is hardship for me. I want my funds applied to bill I received. I want copy of bill that funds used to pay. I want my preferences to have hard copy invoices mailed to me.

      I was advised by my *********** as well as my ************ to see a primary care doctor for my underlying medical conditions. I went to the nearby healthcare center to find a new doctor and make an appointment. But the office was closed. From the office window I collected their phone number and called them on the following day July 08, 2022 for making an appointment. But nobody was answering the phone call. The automatic voice record was advising me to go onto their website and make an appointment other than waiting on the phone line. After waiting about half an hour, I went on to their website, https://doctors.lifebridgehealth.org/********. From the list of physicians I choose **. ***** ***** ****, ** and made an appointment with her for Aug 01, 2022. I received appointment confirmation to my email as well in my cell phone via text messages. After that confirmation while I was waiting for my appointment date, I received several communication from LifeBridge Health Medical Group about reminders, reconfirmation and they asked for my personal information, insurance policy information, etc. On the day of my appointment, I took time off from my work, drove myself, and arrived at the doctor's office before the scheduled appointment time. When I reported for check-in at the front desk counter, the agent checked my name and appointment confirmation, then asked for my insurance card. Then she said, "You cannot see the doctor, because Dr. ***** does not see any new patient." I said, "If she's not seeing any new patient, then why you are giving appointment to us. I need to see a doctor. That's why I checked your website, I saw she's accepting patient, so I booked an appointment. Now after three weeks of waiting when I came here, you are saying that I am not going to see the doctor." The lady name's Tamara kept arguing with me adamantly saying, "You cannot see the doctor today. You need to make an appointment with some other doctor if you need to see someone." See attachment

      I had surgery on 08/31/2021 and was billed by LifeBridge Health for $7,292.63. My insurance company initially denied the claim citing an injury form that needed to be completed. The claim was later approved and Tricare Prime (Humana Military) paid the agreed upon amount of $5,950.35 on 04/29/2022. During the months it took for Tricare to approve and pay the claim, I agreed to a payment plan to avoid being sent to collections. I paid a total of $923.43. I am now due a refund because my portion should only be the co-payment of $63.00 as stated on the explanation of benefits (attached). My overage is $860.43 and I have called Lifebridge multiple times to ask when I will be refunded and each time I call, I am being told an adjustment should have been done but hasn't and they will just note the account. I was told 4 different times that someone will call me but it's been several weeks and no one has called. I've called them 2-3 times per week and each time I'm told there is nothing they can do but note the account and send a message to the refund/adjustment department. I've asked to speak to a manager or the refund department and continue to be told there isn't anyone available and the refund department does not take calls. I've talked to Ms. ******, Michelle, Denise and Charles (the other names I did not document.) I am seeking a refund for $860.43.

      Billed for services not rendered on 12/31/20. I cancelled a doctor's appointment several days prior to the actual appointment. Confirmation of the cancelled appointment was given to me in writing. Payment in the form of a check was accidentally sent to the provider by me. Seven months later, I received another bill for the appointment. I disputed the bill, as services were never rendered. I received my original check back with a letter (dated 8/18/21) from Dawn ******** that stated there was no balance due. There has been no additional correspondence from the provider. On April 27, 2022, I received a phone call and written notice that a debt collection agency (Receivables Outsourcing, LLC/ PO Box 62850/ Baltimore, Md 21264) is trying to obtain payment for this appointment from me. I contacted Dawn Micciche at Lifebridge Health on April 28, 2022 and left a message. I have not heard back from her. I restate. I do not owe this debt. The services were not rendered.

      I received a TeleHealth visit on 12/17/21. I was told there would be a $35 copay and provided my insurance information. Lifebridge Health failed to bill the insurance company provided. A year later I received a bill in the mail. Lifebridge has done nothing to reconcile the issue after multiple phone calls and insurance information was provided again. Lifebridge told me to contact my insurance company. My insurance company said they never received any information from Lifebridge and because it was greater than a year that they were no longer obligated to pay the bill. So now I owe Lifebridge $445.

      This dispute involves a payment made to Lifebridge that i was not credited for. After multiple phone conversations with the billing department i sent an email to a Lifebridge employee with a copy of the canceled check that clearly showed my account number and date the payment was made and check was deposited by Lifebridge. The response i received from Lifebridge was: ACCOUNT WILL BE REVIEWED FOR POSTING OF PAYMENT I have copies of all the correspondences and can also provide Lifebridge employee names to BBB if requested. Its been 3 weeks now since i sent proof of payment, and now i have received a mail notice from the debt collection company Receivables Outsourcing LLC. i would also like to add this is the second time Lifebridge has double billed my family. A previous bill was for $30 and we gave up disputing it because of the relatively small amount. I would like to request intervention to resolve this issue and have Lifebridge address the problems with its billing department.
