8/29/2022: I knocked out a front tooth and loosened two others in a gym accident. I went to Bethesda Dental Group (BDG) for dental work. They based the cost on how much they estimated my dental insurance would cover. Before providing care, the office requested I pay the difference between their listed price and the estimated insurance coverage, which my significant other did (receipts attached).10/10/22: I checked the ******* (insurance) claim to ensure I paid the correct amount and wouldn't be charged additional funds. ******* stated that to cover the prosthesis (bridge) placement, they needed documentation that it was an initial bridge and not a replacement. Since 10/10/22, I have called *** 14 times to get them to send ******* the required medical documents and they have yet to complete this. They have consistently misled me on their timeline and stated they would follow up with me and the ******* when they did not. Dates for the two classes of incidents are listed below.1. Asked for *** to send information to me or *******. *** accepted the request but neither me nor ******* received it. Occurred five times (10/10/22, 10/19/22, 10/31/22, 1/9/23, 2/13/23)2. *** promised to contact me by a certain date but failed to do so. Occurred ten times (2022: 10/10, 10/17, 10/31, 12/6, 12/14; 2023: 1/9, 1/13, 2/21, 3/6, 3/8)Details about each of these incidents are listed in the attached document. Calling *** and constantly having to discuss the incident has been emotionally distressing as getting my front teeth knocked out was a traumatic day. My goal with filing this request is to push *** to submit the necessary documents to ******* and ensure ******* has all the information they need to finish the claim, so I can confirm I paid the correct amount and do not owe any more money.
