My name is Gerald ******* and live in Tucson, Arizona. I am a City of Tucson Retiree and have been an Avesis Vision Care Plan Member for years. I have always paid my quarterly premium payments with check upon receiving the invoice and payment coupon for $15.15. Today (03/28/2023) I received a letter informing me that my vision coverage has been terminated as of 01/01/2023 due to non-payment of premium!? I have not received an invoice or payment coupon since 09/16/2022. I will sign a sworn affidavit and take a polygraph examination proving that I did not receive the latest invoice and payment coupon. If my vision coverage was terminated as of 01/01/2023, why didn’t I receive a letter, email, or phone call prior to termination, alerting me that I was in danger of my vision coverage being terminated for non-payment? Furthermore, why did avesis pay $35.00 towards a vision exam performed on 02/02/2023? and $93.00 for lenses and frames bought on 02/11/2023 when coverage was terminated on 01/01/2023? Why did it take almost 3 months to send me the termination letter? I am on a fixed and limited income and I did nothing wrong to have my vision coverage terminated. I contacted an avesis representative today and her name is Jinika and she is based in Phoenix. She claims that she could not bring up anything on her computer, put me on hold for several minutes, then finally stated that she would have to transfer me to the City of Tucson Benefits Office. She knew full well that any and all city offices regardless of the state close at 5 PM and it was after 5 PM. She made it crystal clear that she was finished speaking with me! I’m asking that you please assist me in getting my vision coverage re-instated.
