I was seen by *************** for a Neuro Psych Exam after sustaining a TBI. *************** asked me about my education and employment background but didn't do a psychological Assessment of his own. The report he wrote was based on a psychological Assessment from 2014 when I had gastric sleeve surgery. Almost ten years ago as pointed out by my own therapist at ******* and Associates. I had done extensive therapy since that time and had been able to move past the psychological issues that plagued me in 2014. But *************** didn't write that because he didn't take the time to do his own psychological assessment. Because of *************** not doing his job properly I am now having to deal with caregivers who depend on ******************* inaccurate statements of me when seeking care and not being listened to or given the benefit of the doubt when cared for by other health care professionals. It also greatly affected the outcome of my case against a distracted driver. I never asked and still don't ask for favors. Just accuracy and fairness but don't know if that will ever happen due to ******************* laziness and pure arrogance. My therapy worked but *************** never took the time to find that out. If they don't believe therapy works and patients don't heal and change why do they tell patients to go.

This was in August 2021. These guys are total crooks. I ended up in the ** last year while working out of town with my 14 year old son. They wanted to kick me out of the ** despite being in incredible amounts of pain and not being able to walk. They finally said they could admit me, but my 14 year old son would have to leave. Mind you, this is at 3am in a crappy part of town, while there were active protests happening...... after multiple attempts to make them see reason, they said that the only way he would be allowed to stay is if they admitted him as a patient. This kind of blackmail is criminal. Now, I've disputed this twice. But they just keep sending me bills. I was on so many different pain medications that I definitely wasn't in the frame of mind to really process this blackmail. But it was either endanger the life of my 14 year old, or let them bully me into a $4,262 bill. Absolute garbage. I'm about ready to *** them for blackmail, and for attempted child endangerment

Please share the name and date of birth regarding the person this concern is regarding.  Your notice lists the name of the father, but the notice is clear in stating this is about the son.  This is important to have in order to provide an appropriate investigation.  Thank you.

I was injured at work on February 25th, 2021. I went to Hennepin County Medical Center to be treated. This is a Workers Compensation Claim. I have never had medical care at HCMC that was my responsibility. I have had 6 visits to HCMC that are all the responsibility of Workers Compensation. I have been working with the billing department at the hospital since February to get them to **** work comp instead of my personal insurance and asking for out of pocket payment from **** ended up paying out of pocket for my take home medications as the **** was about to go to collections per HCMC billing department. They have failed to fix the issue. They have received payment from my insurance and work comp for one visit. They have put the funds in different Guarantor number accounts and can't figure out how to fix it.

I came in for a COVID test on 12/17/2020. This left a balance of $250 that became patient responsibility. UHC confirmed COVID tests were at no costs to patients and there was a coding error on HCMC's part. I reached out to HCMC regarding this and spoke to multiple representatives regarding this which all said "yea, that's wrong" or "that's weird" and promised to correct this. I received last chance calls and letters regarding the outstanding balance and I spoke with representatives on multiple occasions upon receiving those communications and was assured I would NOT be sent to collections and they would correct it immediately. I learned I was sent to collections from a letter sent from the agency. I reached out to HCMC again and was greeted by the same representative that had assured me I would NOT be sent to collections. She apologized and said this would be corrected immediately. I am still in collections with no resolution and promises of a supervisor contacting with no follow throu
