      I have Two people that are schedule to come out and assist me with my problems twice a week. One of the individuals comes out as scheduled but the other individual does not show up. He does not call to notify me that he is not coming. He has not seen me in three weeks. I talked to the other individual that show up and he stated that he would talk to his boss about the other individual not showing up. ***** is his name that does not show up for his shift. The services they provide are making sure I take my medication, they talk with me, grocery shopping when they are able to , and if they notice something is wrong with me they notify the nursing home to make sure that I am getting the services that my insurance pay for. I would like for them to come on the days they are scheduled to come and when things are wrong with me they notify the proper people.

      I am the Assistant Executive Director for Adapt of Missouri. That program is a mental health center. The person making the complaint is a consumer and is currently in a nursing facility. Our agency provided services where we go in to the facility to check on the wellbeing of our clients there under a contract with the Missouri Department of Mental Health. I am the supervisor of the staff person for whom the complainant is making the complaint about. What I propose to do is to go out and personally interview the consumer and speak to the stand of the facility, to see if my staff had signed in and was recognized as being present. I will also check his service notes for notes that he has written about the services he's provided to the client. I will also check our system *****, which is an electronic tracking program tied his cell and GPS, to see where he has been. I will then report that to my executive director and our company's  quality assurance legal department, and they will report back to the Better Business Bureau. I will be going out on Monday 5/15/23.
