      I emailed the clinic a letter to the office on January 18th, that was needed to apply for employment with the ********** of ****** After going to the office several times to try to obtain the letter, I was contacted by ********************* regarding the letter on February 6, 2023, who asked what the purpose was. This was all explained in the initial email. I explained again, and he asked did I provide paperwork from my previous doctor. I explained that I brought in my paperwork from my last doctor that listed all my medications on my first visit. He told me that I needed to go back to the doctor who diagnosed me in *****, ** where I no longer resided. I explained that made no sense and that I had provided them with documentation from my previous doctor and that they had in fact treated me for the issue and prescribed medication. I had been prescribed Adderall but was unable to get the prescription filled due to a nationwide shortage. I was then provided Focalin 2.5mg that was not working and then prescribed Focalin 5mg all at Desoto Family Clinic. He then screamed that I was questioning him because he is *****, although I am also Black. He was screaming loudly and very rude. He told me that the letter was a fraud and he was not going to complete the letter and not to call the office or come to the office anymore when I asked for the supervisor. He told me that he was the director/owner and that there was no one else I could talk to. He was screaming, very rude and accusatory and this was in fact the 2nd time that I had an issue with him. I called back to try to reach someone else and he screamed don't call this office again or come back here. The level of unprofessionalism is unheard of in a clinic. I have never in my life been treated in this manner by a medical professional. I requested a copy of my complete medical records on the same date via email and have yet to receive a response to the email or my medical records as requested.

      Unfortunately we do not write recommendation letters base Providers Diagnosis. She stated her diagnosis was from another provider 3 years ago. We offered her to go to a specialist to get a more current diagnosis and she did not want too. She continue to tell us on how we should do our job demanded that we write the letter. I also explained, if we write letter based on  another Provider diagnosis that would be fraud, because we did not diagnosis her. 

      We can write the letter but, this patient need to go the specialist that we recommend. Once the specialist diagnosis her, the specialist will write the letter based on his finding and not based on what the patient is telling us to write.

      This patient also was trying to get us to fill out FLMA paperwork for a job that she has now, which is contradicting the letter of trying to get a job with Department of labor. If you are asking for FLMA at job you already have and working at another is Fraud.

      I am rejecting this response because: ********************* is a complete liar. They gave me the referral to Pychamore, LLC and I went in for the appointment on February 3, 2023 and started treatment on February 8, 2023. (see attached documents. I did not demand anything, I asked why couldn't they document the **** when they had already been treating me for **** and had in fact written a letter to my current employer letting them know that they were trying to change medications and provided me with a 3 day leave. In addition, this is not a referral for employment, it is documentation of my **** to apply for a job with the ********** of ***** under the Schedule A designation. They have prescribed **** medication for me 3x. They did not ask for me to be seen by a specialist PRIOR to prescribing the medication in December, January, and February. Was that fraud?
