      Was Referred to *********************** clinic by my health care provider as ***** Health neurology was booked out more than 3 months and I was in high pain level. At initial visit 09/13/23 she said she could help me & advised me to start that day to try it (no mention of cost). She said her biopsy method was needed to determine my level of nerve damage. I agreed to biopsies and try 1 treatment (I also ordered a home TENS unit that I paid independently out of pocket thru another provider she referred) When I checked out, the receptionist scheduled more visits, then advised I owed $500 upfront + $4345 due next visit. I was unprepared -****** should have disclosed terms during consult before treatment. I paid the $500 and said needed time to think it thru. *I did not sign any payment agreement. I called **** stating could not afford & cancelled appointments. I stopped in 9/23/23 with new TENS unit to learn how to use & I advised LH neurologist scheduled new ************* asked me to drop those records off when available, she knew I had no further visits scheduled. 10/23/23 I dropped off EMG/MRI results with receptionist for my file. I didnt expect to visit with ****** but she saw me standing there and brought me into her office, tried to convince me to continue treatments with her. I didnt commit and left (shocked she charged $91 for that!) I just received $181 invoice and tried to call, phone was out of service. Drove to her office to request $320 refund (difference due from $500 paid). The office was locked, I could see thru glass doors it was empty. The adjacent office said they moved out in middle of night 2 about weeks ago. I want to dispute this invoice *duplicate charges I overpaid upfront & no way to obtain refund due me. I have impeccable FICOs, concerned this unregulated invoice may be turned into collections. It should be noted that the invoice has no contact phone number and invalid mailing address (abandoned office). Thank you.

      The provider called me today. She had to close business due to out of state family emergency. We agreed she would retain the $500 deposit (she understands that I did not sign anything stating that I knew it was no refundable) and I do not owe the billed $180.86.
