i went to get a cracked filling repaired around a broken tooth at the ********* office and was told up front how much it was cost by the front desk man and later by the woman dentist in the back . they both quoted me a price which i paid that day before having the work done . after leaving the office the next days i got very sick which caused me to not be able to move due to extreme pain this lasted for around 2 weeks . about a month later i received a bill in the mail from commonwell health for the amount of ****** and the amount of ***** and ***** which i did not agree on. i went back to the ****** office and was told it was because i didnt file a no income report which i did when i first visit which caused the extra charges. first he said it could not be filed because it was past 30 days from the visit . then i explained to him showing him papers that he missed up my name and account & had me in two separate accounts . one under ***************** which the paper work was done and under ******************* which then he said he would have to talk to his supervisor to correct it which he did not fix because i still get these bills in the mail .. later i found out the tooth repair she did was incorrect and it broke around the filling which cause pain and a bad taste in my mouth from the crack . the filling she did lifted off the tooth which has caused me to have to go back to another dentist and pay to fix her mistake . am asking for a refund account ******* *******
