I need a patient assistance application filled out. The doctor has one simple page to fill out. I have been back and forth because it's missing stuff/incomplete application. It's the application alone, not the prescription he wrote. I have been back and forth and finally we got things filled in. Why he is complaining that it's time consuming, it's one freaking page. Just the providers name, address, telephone #, the name of the medication, strength, and quality and description on how to take it. Then his signature. Why he is complaining and blowing this out of proportion is beyond me, it has to be filled out by the doctor. I may need a new form completed because the original looks awful, just fill it out like it ask us to then I can fax or mail in. I am very angry because no one wants to take a few mins to do so I can get this medication at no charge. Linzess is very expensive. Another doctor at the same facility filled out another application for a different drug no problem at all, received that medication already by mail at no charge.Business response

Business Response /* (1000, 6, 2022/03/30) */ We are regretful to learn of this patient's experience and this is not the experience we strive to deliver. We have a centralized Medication Assistance Program that completes the necessary paperwork to help patients obtain medications from pharmaceutical companies. Unfortunately in this case, despite our best efforts the completed form was not accepted by the pharmaceutical company. Once we were made aware, we reached out directly to the pharmaceutical company to determine why the form was not accepted. We immediately corrected the information and resubmitted the form so the patient could receive their medication. We have directly apologized to the patient for their experience and additionally have provided re-education to staff to prevent reoccurrences.
