I had called CHI for a financial assistance form for both my daughter and I. They sent a hipaa form request instead of the financial form. They then sent my *** **** *** daughters account in to collections. When I called and tried to resolve this they told me I requested the HIPAA form not the financial request form. Why would I call the financial assistance line and ask for a hipaa form. When I called I spoke to both a representative and supervisor (6.5.23) and neither of them seemed concerned that this is going to impact my daughters credit. I have not much hope that they will resolve this issue.Business response

You have qualified to receive a ***% financial assistance discount. The accounts in question are now considered Paid-in-Full. Please find copies of your approval letters enclosed for your records.
