Chronic Care Service Methodist Physicians Clinic (###-###-####). I was told by the provider representative that this service is cost free. I received a billing statement this week. I followed up by calling the phone number listed for the past two days. I have gotten zero response. I want to drop this service.Business response

We are deeply regretful that our billing practices regarding this service were not well communicated. Participation in our Chronic Care Management program has been cancelled per your request.
