      They have way too unrealistic expectations of someone in my situation. Out for 3 or 6 months? Sure, understandable. Paying over something like $2000? Sure, understandable. This is literally only $591 total. Just one month of payments is all. Why am I filling out paperwork for total disability? Why do I need to have 4 different people filling out forms for one thing? There are no alternatives I can send in to cover this. I'm not comfortable with my boss filling out anything to do with my personal health, especially knowing how he could easily falsify something. Car insurance doesn't require this much work for a claim. Health insurance certainly doesn't require this much. Homeowner's doesn't even require this much for a claim. I'm not asking for permanent assistance. I'm not totally disabled. I was out of work for a month. That's it. It's seems as though they intentionally make it impossible for someone in my situation to get the help they need and easily.

      A detailed reply was sent directly to the customer. We appreciate the customer's comments; however, to appropriately review the request for disability protection, we need to receive fully completed forms that were previously sent to him. We need information from him, his employer and the physician who certified to disability. The benefit request form includes separate sections that are intended to be completed by each of these entities. If the customer is concerned with his employer having access to personal information, each section of the benefit request form may be completed independent of the other. Alternatively, we are happy to contact the employer directly to obtain the information we need; however, we need to receive the employer's name, along with contact information. To date, we have not received any detailed information from the customer regarding his request for benefits.

      As indicated in my October 13th reply, a copy of our October 12th letter was emailed to the customer. Following his receipt, he directed the attached email to Central States Health & Life Co. of Omaha (CSO), which was received yesterday. In accordance with his request, going forward, copies of all letters sent to him will also be directed to your office. If you have questions or concerns regarding this, please let me know.

      To date, the insurance company has made no attempts to assist me in this matter in any reasonable way. The expectations they have are unrealistic. I could have the surgeon fill out a form, but I do not trust my employer to fill out any form related to this. Not just because he does not need to see my health information, but also because I do not trust him to fill anything out truthfully and honestly. On top of that, I still find it unreasonable to request so much information for so little. If the credit union has reached out to them already on our behalf, that should tell them something about the seriousness of the situation. The form of proof regarding my employment situation I'm willing to offer is pay stubs from the past month - or rather the lack of pay stubs. I'd even be willing to offer bank statements showing the past month to support the fact that I have received $0 from work the past month. But my employer will think he will owe something, or he'll lie because he knows I've accepted a new job. They need to work with people in this situation. It's unfair to us and I am very concerned about my claim being denied because of my employer not being honest.

      Requesting the file record for my entire claim ******* as I was forced to have a family member take over the car payments who plans to pay the vehicle off and take over the title for the vehicle when paid off soon, I am now without a vehicle and it has created a significant hardship to maintain my healthcare . CSO needs to provide by mail the entire record of claim ******* as I will be seeking legal counsel once I have my records of the claim.
