I was admitted to Avenues Recovery on 09/06/23 through 09/12/23. Both Avenues and my health Insurance provider (******* ************** ********** ****** made errors on my acceptance to Avenues and due their mistakes, I had to leave on 09/13/23 due to health insurance coverage issues. This was a 30 day voluntary program that ended after 7 days due to errors made. I was told by Avenues that my stay there would be "No Charge" due to the error. However, I was made to sign an Appeals Form so Avenues could contact ******* ******* for the purpose of getting them to pay for their portion of my stay. ******* ******* did pay Avenues a portion. I have contacted Avenues "Multiple" times asking for them to send my an invoice stating that I personally will not owe them any money and they "WILL NOT" respond back. I received an Explanation Of Benefits from ******* ******* and it states that the portion I owe Avenues is $5455.70 (my deductible). Avenues is doing something that I believe is completely unethical and will not send me any Zero dollar invoice or a letter stating my stay there is "Paid In Full". Avenues is a substance abuse facility that I went to voluntarily for a 30 + day stay and had to leave after 7 days. The mistakes made were not corrected and I left that facility full of frustration and anger considering I went there to get well with my problem.
