      Dates of service from Center for Sight: 11/16/17 to 07/13/22 Dr **** ****** *** S ******* ***** Suite *** Las Vegas, NV XXXXX Referral from Dr ****** ********** I started going to Dr ****** for glasses also had surgery on 9/28/21 Another surgery 10/21/21 Another procedure 5/5/22 I have attempted to get glasses numerous times between first visit and to this date.(7/13/22). They have previously made numerous mistakes on all lenses. Tiffany, who fits frames lost the lenses to my old glasses and when she sent them for replacement she filled the old prescription lens for my new frames with the old prescription . I wore them for 2 weeks and called her advising I could not see clearly. She told me to bring them back. I have not received my old glasses ( which were very expensive) or my new frames and lenses yet. When I last talked to her which has been approximately 3 weeks ago I asked her if she would just credit my account and I would go elsewhere. She said no it's been too long. The Drs would not listen to what I was feeling or what I was seeing . I felt as if I was imposing on them and they really didn't care. I saw one Dr on 5/5/22 who did a YAG procedure and he didn't introduce himself. I asked him his name and he said Dr ******** He told me I would never see clearly. Very rude and unprofessional. Most of the staff were unprofessional and not at all kind or caring. Tiffany who fits the frames and lenses is very kind and professional. She just does what she can and I do understand. I'm still waiting for my frames and lenses. I am wearing 4 year old glasses. I am a Senior Citizen, over 75. It is appalling to take this much time to get it right! My eyes are changing because I can't get the corrected lenses in a timely manner. I have been very kind and patient, but this is causing me to limit daily activities and causing anxiety and depression since I can't read, sew or see street signs clearly. Thank you for your help. We are coordinating ha full refund for the current glasses order and hope we get the opportunity to serve Ms. ***** again in the future.

      i was to get cataract surgery on my right eye they were going to correct cataract stigmatizim and do a implant. Said i need $2200.00 up front out of pocket $1625.00 for dr $575.00 for facility and the surgery was suppose to be 02/07/22. I paid from my credit card 01/17/22 $2200.00. On the 01/19/22 they said that surgery would not work the toric lens is what is needed but that procedure only cost $1500.00. So you will get reimbursed for the difference. Then Jeannie called and said I would have to pay $1500.00. before i could get surgery so on 02/26/22 i cancelled. I still have my cataract and never had surgery. and they won't give my $2200.00 back on my credit card. They had it over a month. I have all paper work if you need anything to back up my thing.

After the initial exam for this patient, it was discovered that the option originally selected was not necessary and the patient would receive the difference in payment back ($2200-$1500=$700) via refund. No additional payment for the IOL option selected by the patient was needed to proceed with surgery. The patient did have a deductible through their insurance that was to be met before proceeding with surgery. Upon the request for the deductible the patient chose to cancel the procedure and requested a full refund. In order to process the refund, our staff requested the credit card number used for the payment on 2/1. The patient refused to offer that information and informed us that they would be contacting their bank for a dispute. On 3/10, the credit card dispute was completed for $2200. We would be happy to schedule a reevaluation in the near future and help this patient through their cataract procedure. Consumer Response /* (3000, 16, 2022/04/06) */ (The consumer indicated he/she DID NOT accept the response from the business.) on Jan 19th Jeannie had me back in office to sign papers for toric implant because they could not do the $2200. one i signed paper for $15oo. one she said i would get the difference back. on the jan 21 Jennie called and said there is no record of the $2200 and i would need $1500 for the toric surgery so i canceled the surgery. Not because of deductible you said no addition payment needed to proceed with the surgery. *** said on 04/04/22 they have not received a response from center of sight and not to give them credit card number Sandra said the refund would take 30-45 days and she needed to talk to her supervisor. And i can share it with my bank account at boa. They have stated with severial attempts you have not responded once. The only thing i want is for you people to quit making people go through what you have made me go through these last 3 months and telling them insurances does not cover it. We contacted ****** who handled the chargeback dispute and they provided us the attached. They informed us that the patient would have received the money back on her card ending in **** when she initially contacted her bank to file the dispute which was on 2/19/22. The money was debited from our account on 3/9/2022 as you will see on the attached Dispute Notice. These funds went back on her Visa ending in *****
