      On Or around July 17th i went to 505 dental, i discused getting a new upper ****** which was supposed tp be paid by my insurance and he said i needed an implant. They said your husband was granted a koan of up to $8000.00. After Pondering on the the amount $5700 I told them I don't want to do it. They tried to force me telling me he already ordered the implant which is a lie because i never had the ct scan which needs to be done for an implant. They already took the money out and my house denied the partial which was the reason for the implant in the first place. I've been consistently trying to get them to return the money to the lender because I didn't have anything done. 5 months later they still haven't returned the money and the bank is calling constantly and they didn't do the work. They're keeping the money. I've encountered such unprofessionalism. I spoke to him and told him I'm not getting the implant and he said he said ok. 3 days later he acted as if we never spoke about this. Bottom line they took out a loan without doing any work.

      Patient was examined for implant treatment on 7/10/2023. Patient was presented treatment plan , accepted treatment . Applied to third party financing payment plan, patients partner agreed to finance patients treatment plan. Patient was informed she will only receive a partial refund for treatment as indicated on the signed disclosure (30% broken contract fee). Patient was informed in order to receive partial payment her and partner will have to come in to the office to sign the refund paperwork when owner is in the office. To approve partial refund. Patient refused partial refund and has not come into the office to sign paperwork to receive partial refund.

      On October 6th, 2022 At Around 2:00pm I ************************* Had an appointment to get fit for dentures. After i was done The finance Employe Name "****" came to talk to me about snap on dentures he said i would get a free set of top and bottom dentures he told me about the snap on implants for the bottom but i did mention about the top he said going to be both for top and bottom $575.00 with interest what proceeded this and what surprised me was he had a contract already made up because i showed interest from a previous visit what troubled me and perplexed me was that he showed me a contract with lending point on my phone i was about read the contract but he said "let me see your phone" he grab my phone without my permission and checked ok on everything on lending point contract electricity my son and my son girlfriend witness "****" completing the contract without mom permission. i also dont understand how he got my personal information like my social security number that was not given to him and other information to him. he did not go over the contact with me or go over any term that have to do with the payment and the total amount and he also made me sign me a paper from the dentist without explaining everything to me and rushed it to get my signature.

      This complaint could not be further from the truth. ******************* had been in the office previously and spoke to one of our doctors who spoke to her about "snap on" upper and lower dentures.

      When i met her i told her to start with the bottom snap on denture which is where its typically more needed but she insisted on both the top and bottom snap on dentures. I explained to her the price , the process and all her finance options.

      She was quite clear as to what she wanted. Additionally, her claiming she didn't know how I knew her personal information such as her SS# ect is erroneous. In fact, when ******************* left our office she was extremely excited to have teeth again.

      To date ***************** has gotten her immediate dentures. When she heals she will be ready for her implants. This office to prepared to complete all the treatment in her contract. In addition, she has since decided not to do the upper snap on denture 

      and thus our office has already refunded $8,000 to her account.

      Lastly, ****************** apparently has a boyfriend who came to our office in an attempt to intimidate us.

      This person is not our patient nor is he legally connected to the patient and therefor any discussion with this person regarding ***************** treatment is a Hippa violation and illegal in **************.

      I want this on record in case this issue moves in a different direction.

      On June 20, 2022 I was seen at 505 Dental and was advised that I needed invasligns and implants. I was also informed that my insurance would cover most of the cost of the invisaligns and I would have to do financing for the implants as they would cost about $12,000. My application for financing was not approved but the representative ***** suggested I get a co-signer to help with the application. I reached out to my mother and she agreed to be a cosigner on my application and the loan was approved. When I was originally at the office the owner of the dentist office and ***** informed me they would be very flexible with my schedule since I informed them I work for the court and it would be extremely hard to get weekdays off. Shortly after my initial visit I was informed that the specialist who needed to see me only works on Monday. I called the office on numerous occasions to inform the dentist that this would not work with my schedule and at that time ask to have my agreement cancelled. I was told ***** would get back to me which he never did. A few weeks past and I received a call from my mother stating she received a call from a loan officer stating that she was in default of her loan agreement. I never agreed to have the loan put in my mother's name and when I found out what was done I tried to have the situation straightened out , after weeks of getting the run around from 505 dental I came in to speak to a manager, owner or ***** none where available. I proceeded to call the office daily until a manager by the name of ******* called me back and informed me ***** would be the only one that could help me. She also stated he is only in the office on Mondays and Wednesdays. I returned to 505 Dental on Wednesday September 14th to finally speak to ***** who informed me he would not remove my mother from the loan and he would not cancel the agreement. He stated I needed to pay over $10,000 that day in order to have my mother removed to cover expenses.

      To Whom it may concern:

      As per her complaint, ********************* entered into a treatment plan with our company on or about 6/20/22.  Her treatment plan included Invisalign as well implants, abutments and crowns.

      ********************* first complaint is incorrect. She claims that invisalign was mostly covered by her insurance company.

      In reality the invisalign treatment is $5,600 and her insurance only covers $1,500 of the treatment

      leaving her with a balance of $3,760.  

      She then states that the specialist that she needed to see was only available on Mondays. This statement is false and in fact, our specialists  are typically available 5 days a week and we work hard to accommodate all of our patients schedules .

      She then states that ***** was only in the office on Mondays and Wednesdays, when in truth ***** is in this office Monday,Wednesday, Thursday and Friday and is always available by phone. 

      The patients claims she never knew her mother was on the loan. Obviously she knew as her mother was on loan as she supplied us with her social security #, DOB, Address , cell phone and email. 

      So her claim is without merit.

       The reality is ********************* entered into a contract with our office to complete dental work. In order to pay for the work she enlisted her mother to help with the financing as she didn't qualify for our financing program.

      Now, 3 months latter, after we completed the invisalign lab work , contracted with the specialist to complete her implants and ordered all the necessary supplies to complete her work she now claims she had no idea her mother was on the loan.

      This is obviously a ploy to get out of a signed contract which is available upon request.

       I never stated my mother was not on the loan I stated my mother is the only person ladies on the loan, my mother was supposed to be a co-signer only. Secondly I have messages where different staff informed me the specialist I needed was only available on Mondays . I work for *******************, I shouldnt have to put my job at risk because a dental office will not work with my schedule . I am pretty sure they keep a phone record and I have called numerous times trying to get an appointment that works with my schedule. Obviously I would be able to make payments if I entered into the agreement. The fact that I couldnt even get someone to speak with me on the phone and had to come into the dental office on my breaks on several occasions proves they are a dishonest business . I entered into an agreement not my mother no one from the lending company nor the dental office contacted me about payments , I even supplied my debit card to have payments deducted from my account which never happened . This whole situation has put a damper on my relationship with my mom because it seems like Im a liar. I feel like I have been swindled. I am asking for my mother to be removed from this account because its not her loan its mine.  I would have never agreed to put a loan in her name. A co-signer is very different than the actual account holder.  But when I finally was able to speak to ***** in person he informed me that me and my mom were on the hook for this loan but thats not true its only my mom and thats not fair.

      All of the patients concerns have been addressed. She admits that she scheduled the treatment .

      She objects that her mother is the sole person on the loan and that she is not on it as well.

      The patient is not on the loan as she simply did not qualify for the financing , hence her mother signed the loan on her behalf.

      If her mother didn't want to help her daughter why would she have given all her financial information to proceed? Whether a co signer or

      an actual borrower both parties would be responsible. Clearly there is no difference. I submit that this issue is between mother and daughter and has nothing 

      to do with the patients dental care.

      If the patient wants her mother off the loan we will oblige as long as she pays the loan up to date.

      This was explained to the patient several times. The reality is we cannot remove her mother from the loan until the patient either pays us or gives

      us a viable person to take her mothers place.Tell us why here...

      This is so frustrating .Yes my mom sent me her ID to help not get a loan for herself, if that was explained to me I would have never used my mom.  She is an older woman and she doesnt deserve to be harassed. How could I even make payments on the loan if I wasnt even notified . I am getting nowhere with this business and their deceptive business practices. I was never told that I wouldnt be on the loan and now they are not willing to break this agreement. This business claims I have to pay $10,000 in order for my mom to be removed from the loan that makes no sense at all. Why would I want to do business with them, they totally blatantly lied to get the financing. I dont understand why this cant be cancelled!!!! They will not work with my work schedule like they assured me they would and if I am paying for a specialist they had to pay then why isnt that specialist only available on Mondays.  Why isnt the specialist working with my schedule since they want $10,000. To be short this isnt fair. I had been coming to this office for years before all of these new associates were hired and Im simply appalled at how this is being. I handled. I feel like a complete fool just for wanting to get my teeth fixed. I dont even feel comfortable setting foot in that office ever again.

      They made me sign a contract and they gave false information. I never realized it was a credit care and they will charge me 28% of interest. In addition, I didnt provide any ID or social security in order to sign a contract. The person who did this contract made false statements and knew I was unemployed which I am not able to afford. I call the office in order to cancel my payment plan and they said I cant cancelled. Also, they used the whole money in the card without performing any of these treatments.

We asked ************ ***** if she would like to apply for our financial program we offer here for  dental  treatment not covered by her insurance . ********* ****** explained to her our  financial program in detail. *********** ***** agreed to the terms and conditions .
