On **** 1st I contacted City dental to confirm that they were in fact in network with ********** the representative confirmed they're in network via telephone and requested my ********** information. I then provide my ************* group number which was accessed by City Dental tax ID on **** 1st, 2022 for my initial appointment scheduled for **** 2,2022 x rays and cleaning. Which today July 5,2022 I now see has an claim number ******** assigned to company **************. After receiving cleaning and Xrays on **** 2nd I was told to return **** 3rd for fillings ************ and unknown last number. I returned On **** 3rd for an 1pm appointment 3 fillings on the left side of my mouth 1upper 2Lower. Upon finishing the appointment Billing representative **** informed me that I had an remaining Co payment for fillings. Below is the receipt provided to me by City Dental billing department representative ***** In reference to Claim number ******** which is also filed under unknown company Tax ID Uptown **********************. On **** 6th 2022 I contacted City Dental to file a claim on the $695.25 payment. Billing Representative **** for the first time informed me that City Dental is no longer an active business and that There new name was ************** an out of network dental office. Which at the time was Unbeknownst to me the tax ID used by Representative **** for **** 3rd copayment of $695.25 instead of City Dental tax ID for my co payment $0. When I asked Rep **** why didnt She inform of the sudden change in the companies name and network. She rudely stated and I quote its ********** responsibility to inform you of those changes not mine. I immediately contacted healthplex and was assisted by representative Pree and her manager. However, on At no time prior to **** 6th 2022 did she or the other front desk employees inform me of City dental no longer being an in network provider. Obviously City dental/ Uptown medical are actively scamming new customers out of their money.Customer response
