I received my statement dated 12/11/21 on or around 12/20/21. The statement showed that I owed $131. 43 for 9/2/21. I called the company because I had already submitted payment by phone and the payment was deducted from my bank account on 9/29/21. I spoke with a supervisor (*****), several times in reference to this mistake. During our conversation, he reviewed my account and I gave him the information that proved the payment had been deducted from my bank account for 9/2/21 on 9/29/21. ***** then stated that they had changed my policy, but they would correct the error. I requested at that time for him to issue a letter stating that my policy was never in default and that there never should have been a change to the policy. them anything, because in spite of him telling me not to submit my payment for 12/2/21, when we had spoken, I mailed the payment and it was deducted from my account on 1/4/22. I asked as of 1/26/22, what was the cash value of my policy and ***** the supervisor stated that it was ******* dollars. I specifically asked if I requested to surrender my policy, would I receive the ******* dollars that I was entitled to, and he stated yes. As of 1/26/22, the surrender form was faxed to me and I completed it and faxed the completed form on 1/27/22 at 8:30am. I also mailed the form using priority overnight mail, with a signature request on the same day. I spoke with **** and they stated that they received my fax on 1/27/22. The post office tracking also confirmed that the letter was received by *** on 2/1/22. I phoned *** on 2/7/22 in reference to my surrender request. I was told by ******* that my policy was cancelled on 1/26/22, and that they mailed me my payment of $131.93, which I have not received as well. I was told that the policy surrender amount was $******* and that is what I am entitled to because my policy was in good standing at the time of my request.
