I originally called and asked if they took my insurance ****** ****** **** **** - They said yes.
Then went in for a plate for 2 teeth on top that were pulled from my prior dentist - who left the facility. Went back @ least 7 or 8 times, sometimes didn't do anything just talk to me. She had no intentions on doing a plate from the beginning, so @ the end of it all right back to where I started no teeth and no plate. Originally she said that my insurance would cover it but already payed 200.00 plus insance would cover same totaling 913.00 with balance remaining 445.00

Patient first arrived for a consult and exam visit on 11/6/23. We received information regarding his insurance then. Although we are in network with ****** ******, we have found out through our predetermination and insurance submission to this company that he has a different ****** ****** plan that we do not participate with. He was informed this at every visit when he came in and that his copayment was expected at each visit since his insurance does not cover 100% of his cost. He came in for a cleaning and discussion of his treatment plan on 11/8; then fillings on 11/21, 12/6 and 12/26. At each of these visits, as well as several phone calls from Larry, we informed him that he has a copay. He promised to pay his past balance after insurance submitted all their payments but then only paid partially towards his outstanding balance on 12/6 for 99 dollars and for 12/26 for 100 dollars. At each visit, we showed him pictures of the teeth that had cavities and the recommendation to fix these before a partial denture could be made. Patient not only agreed but was told of the cost of the additional fillings. He then made no attempt to pay his bigger balance. He was told repeatedly that we were not in network with his insurance but still made the choice to see us on 11/21, 12/6 and 12/26. We have given ample opportunities to pay off his balance but patient states on the last phone call to us on 1/31/24 that he had no intention of paying and threatening to call a law firm and news media. We decided to not give him any more appointments until his balance is paid in full and because of his threats and harassment. It was after we informed of this that he threatened to sue us. The attachments he sent in include a treatment plan that is not something that he has to do, it is just proposed.
