I have been a patient of Dr. ****** E***** since 2001 as he has transferred to different outpatient providers. I suffer from severe PTSD, Generalized Anxiety Disorder, Bipolar Disorder as well as severe Attention Deficit Disorder. I was on Xanax and Adderall for my symptoms. These medications helped me greatly. I was able to maintain full time employment and function in main stream society with the aid of these medications. I found that the medications helped me immensely. Approximately July of 2023, Dr E***** abruptly stopped my medications, stating that he no longer wished to prescribe me controlled substances. As a result my functioning has greatly diminished to the point that not only can I not maintain even part time employment but I find it difficult to even leave my house due to agoraphobia. I cannot focus on a simple task long enough to even follow through with the process of applying for public assistance for sustenance. I cannot focus long enough to maintain a clean and sanitary living environment and my apt was violated by code enforcement in August of 2023, where I was almost evicted. My mind has also become a living nightmare due to my increasing and pervasive anxiety symptoms. I suffer at least several panic attacks a day, sometimes lasting hours. I was even recently hospitalized for stress related A-Fib and a panic attack that lasted 6 hours. I have also developed agoraphobia and find it difficult to leave my house. I have not been able to work since July 2023 due to my overwhelming symptoms and have been forced to apply for disability benefits. I have begged Dr E***** to put me back on my prescriptions again but he refuses simply stating he does not wish to have me on any controlled substances even though I feel my symptoms are so severe they warrant these medications. I am asking for you to communicate with Dr E***** that I need these medications to function and that my mental health and functioning has diminished to severely dysfunctional
