In December 2021 I went into a payment plan known as the **** Plan for $86.00 per month for 1 year which included spay of my dog regardless of size I was told. My dog was finally spayed on March 16th 2022 with forced agreement that I had to sign in order to get her spayed . Then when I picked her up I was made to sign into a payment agreement to get my dog back in the excess of over $500. They left a voice mail stating that they would send me to collections . I also did not receive the Spay Certificate so I can go to the town for her license and show she has been spayed. At this time I would be willing to continue payment on the **** plan however would appreciate the excess fee from the day of spay to be waived and have them mail me the spay certificate.Business response

Client entered into PAW Plan agreement on December 6, 2021 for her dog. At that time she was provided with a Pet Annual Wellness Agreement which outlines on page (1) that patient weighing over 50lbs or with medical conditions may incur additional fees. This agreement was explained by a hospital employee and reviewed by the client prior to signature. On March 11, 2022 a surgery technician contacted the client to provide the estimate plan and cost for surgical procedure. On March 12, ***************************************************************************************************** addition to a signed treatment plan (which details the costs involved with visit). Upon surgery discharge, the client was unable to provide the proper payment for the services rendered on that day. At this point, we entered into a payment plan option with the client which outlined days and payment amount to be made to the hospital. Numerous attempts were made to the client regarding past due balance. All were unfortunately unanswered. Client was placed in collection on April 18th, 2022. On April 19th, 2022 the client contacted the hospital requesting a spay certificate. This was emailed on April 19th and April 20th of 2022 to the email on file. The spay certificate was also physically mailed to her on April 20th, 2022.
