My is ***** **** ***** ****** She has been at this facility for about 3 years. She use to be a nurse for years so she knows the protocols. They don't like when she lets them know when they are doing things wrong. On New Years Eve a nurse snatched a drink out of her hand. Because my aunt did not want to give it to her. All she had to do was bring her a cup. The nurse then told my aunt who was in the bed shut up, slammed her door, and did not respond to her call light for her medications to be given. My aunt is bed bound and is 73 years old woman. She has had several issues with her call light not being answered for over an hour when she needed her medications. My cousins have had to send the Chardon Police department up there at least 3 times in the last 3 months for a well check visit. This is not okay. She has to be transferred out of there ASAP. My children have been lied to repeatly from ******* The Director of Nursing. The social worker ignores my requests. I am constantly being told I am fine. There is a trail of emails with my daughter ******* My medical needs are not being met. My neurologist requested for me to have a MRI back in September and the nursing home keeps making up reasons why it has not been done. I NEED THE MRI NOW. They keep saying I have no payer, but I retired from the federal government, and I have had Medical Mutual for years. They have copies of my card. I need bloodwork, diabetes testing my kids have been asking for since July, August, and September, knee injections I get every 3 months. I have not had those since August. My sanitizer has not been filled in 3 weeks. I am trying to get over having COVID and being incubated. I am 70 years old. I DONT WANT TO DIE HERE. MY KIDS ARE BEING BLOCKED AND LIES TO AND THEY PROBALY HAVE PUT LIES IN MY FILES THAT WHAT OTHER PATIENTS HAVE SAID INCLUDING MY SISTER WHO FILED A COMPLAINT LAST WEEK ***** ***** she is 73, She has been here 3 years and she cannot take it anymore. I NEED TO GET OUT OF HERE ASAP AND BE TRANSFERED SOMEWHERE ELSE. PLEASE HELP ME. PLEASE HELP ME. PLEASE HELP ME. THE DOCTOR IS NOT HELPING ME AT ALL, THEY WILL NOT ANSWER ANY QUESTIONS MU KIDS HAVE. Trying to collect a credit balance ($1095.60) from my late brothers (Army Veteran) account, when he died at one of the facilities managed by Communicare Health 10123 Alliance Rd, Cincinnati, OH 45242. I handled all his affairs. On 8 June 2023, I submitted the paperwork to their Accounts Payable Manager (Eugenia H***). She informed me that they had no record of it, only record of a cashed check in 9/13/2011, for $216.40 made out to him in care of me. I asked for a copy of the cancelled check because I'd never gotten it. No response to that request. The $1095.60 was never reported to unclaimed properties by facility...Unclaimed has no record of it. I hired a lawyer to get the funds, but was told that an Eileen M*****, new to the company, had to approve funds. No record because of an old system "Sage," and because she isn't familiar with it, she is not recognizing the amount owed, though I provided documentation. I've email Ms. H*** three times before I involved the lawyer. I received no additional correspondence once she revealed that the company paid out $216.40 and I responded that I did not get it and asked for a copy of the cancelled check. My brother was a resident of *********** Healthcare Center (formally *********** Health and Rehab Center), **** ******** ***** ************ ** ***** and his account number was ***** (**** *****). I believe there is fraudulent activity involved in both accounts (the $216.40 and $1095.60).Initial Complaint

This place is serving food late, having problems with line and are not resolving the issues. They only have one person in the kitchen and cannot keep employees. The soda machines do not work right. Residents are losing money from this. They treat residents rudely as well.
