      According to three bills I received in the mail, my daughter, ******** ********, had medical testing completed at *************** ******** in ******* ** on October 17, October 18, and October 19, 2023 under three account numbers: *************** *************** **************, for the amounts of $20., $30., and $20, respectively. I am her mother and the guarantor of her insurance and am responsible for paying the charges. There is no indication that these were submitted to our insurance company. In fact, all three bills list "Possible Insurance." The billing website is defunct and I get the following message each time I go to the site listed on the invoices: "Error 403 - This web app is stopped. The web app you have attempted to reach is currently stopped and does not accept any requests. Please try to reload the page or visit it again soon. If you are the web app administrator, please find the common 403 error scenarios and resolution here. For further troubleshooting tools and recommendations, please visit ***** Portal." (See attached screenshot.) I am not confident that my financial information will be secure by submitting payment over the phone.

      ********** used them for pathology services, which I can't confirm, and I need SOPC to submit the bill to my insurer. They are not answering their phone, and they have a non-fuctioning website. They list their address as the same as ******** *****, but ********** who operates the hospital can't reach them. I haven't paid the $50 bill and won't until I get contacted. 3 calls to a Ms. Jennifer T***** at their office in ******** and no return calls. That is why the amount listed is $0

      I received a bill from a debt collector for money owed to Southern Ohio Pathology. I've never heard of this company. I don't know what it is for. Says they are in relation to *********. I contacted ********* about it and they confirmed I DO NOT owe a debt. They confirmed I received Financial Assistance and all of my bills have been covered. ********* confirmed they are unaware of any such company called "Southern Ohio Pathology". ********* confirmed they DO NOT work with any such debt collecting company called "******* ******** ***." We believe this to be a scam.

      In late March, my partner and I both had colonoscopies. This was preventative treatment. Everything is supposed to be covered 100% by insurance, but this Southern Ohio Pathology Consultants listed all scans of the polyps as "diagnostic" rather than preventative like the surgeon, anesthesiologist, hospital, etc., all did. As a result, they are seeking moneys from us to cover the cost of the pathology screening.

      I have two insurances ******** is my primary and ****** is my secondary my claims are going to ****** but need to be paid by ********. I have called 2 times about billing errors by Southern Ohio Pathology and asked for supervisors. The member representatives never have a supervisor call me back or did they file the claims with the correct insurance after they said they would.
