            I had requested Creekside Chiropractic Center, Inc send my records to my current internist and then destroy my records at their office. I haven't been to Creekside Chiropractic Center, Inc in 6 years so this record request complies with Ohio HIPAA laws. On July 6, Creekside sent me an email stating "We will also need a written letter with your request to remove your medical records." I waited to confirm that my doctor had received the records before following up on the removal of my records. After I heard from my doctor, I sent the requested letter to Creekside on July 24. I didn't receive any confirmation that my records had been destroyed, so I followed up two more times on July 28 and August 8. This communication was sent to [email protected]. They were previously responsive via this email address but are no longer responding. I want written confirmation that my records have been destroyed. Thanks.

            We found their billing practices very deceptive. They charged differently every time but did exactly the same manipulations to us. We were told after the initial $100 new patient exam & evaluation, we would pay $20 per visit which we did pay at time of service but then would get a bill a month or more later saying we owed more because it was a different service. My husband's time with the doctor never took more than 10 min. but yet they claimed he had different services and that was the reason he was charged different prices. We were extremely disappointed and felt that they were taking advantage of patients and billing medicare in such a way to get paid by them and patients too! We were told at initial visit that if different services were done it could be different prices but we were never told at time of service that what they were doing was considered a different service and would be a different price. We didn't know it was going to be a different price until a month or more after the service. DECEPTIVE
