      To Whom It May Concern, On 7/2/2022 we bought washer/dryer replacement unit from GE Appliance Store (GE) incl. the installation for my 1-bedroom condo in Seattle. On the scheduled delivery/ installing date 7/16, the team brought the unit, installed, started a testing washing cycle and left. In 30 minutes, we noticed flood coming from laundry room to kitchen area, and we‘ve discovered running water from an uninstalled pipe. The laundry room floor was covered with water. We stopped the washer and called the installing team back. As arrived, they acknowledged that they forgot to connect the drain hose, and connected it. We recorded their acknowledgment on camera. To prevent water damage in the condo below us, we pull the floor in the laundry room, and kept two vents on for a few days to dry the surface We called GE Appliance Store on Monday 7/19, since the office was closed on Saturday, 7/16. GE directed us to Forwardair.com as GE transportation/ installation partner. The claim number ***** was opened on 7/19. After the flood, laundry room floor was not drying nicely. It went with bubbles. A contractor recommended remove the floor and keep vents in the laundry room on to dry the frame underneath to prevent mold development. I few times through July and August I contacted Forward Air to get a name of the person who is assigned to our case. Finally, claim Manager ****** ******** (O: ************* s********@forwardair.com) contacted me on August 26, 2022. I provided her with photos of the damage. I expect her to send their damage appraiser specialist making own evaluation. Instead, she directed to provide a contractor damage repairing quote. The floor replacement quotes I shared is $1,020.00 w/out tax. After submitting to ForwardAir all inquired information, we removed and tossed away all damaged floor materials. I kept contacting ****** in early October inquiring about a status of the claim. On October 10th, we got an inquire from a third-party claim adjuster company with a request to inspect the damage related to the flood claim. We were not able to respond to their inquire, because we removed and disposed damaged floor materials after providing floor replacement estimate quote per ForwardAir request. The floor in my laundry room is not fixed. Thank you for your consideration, *** (*******) ******** Supporting Documents 1. Purchasing invoice 2.       I purchase some equipment for my business the seller offered free shipping and this company was chose by the seller the promised a delivery on a Thursday called me to see if I could unload the equipment and I said yes so I rented a fork lift and paid for a operator to drive it they didn't show when I called them to ask were the driver was they were rude and very unprofessional with me now my free delivery is in Nashville 3 hours from me and there wanting me to drive 6 hours round trip to pick my free delivery up and I will if there willing to pay for my fuel they referred me to my point of purchase my point of purchase reffere me back to them so if I have to go drive 6 hours I will have my attorney contact both this company and the seller

      Forward Air has repeatedly mislabeled our shipments resulting in them being sent to the wrong location. We have to send replacement shipments which cause us to incur duplicate costs from the airline freight carriers. Forward Air in these instances does not hold themselves accountable and denies any financial claims we make against them to recover our additional costs. We have had 3 instances in the past year which have resulted in additional charges in excess of $5,000.00. Their gross negligence and inability to hold themselves accountable is appalling.
