      In January, I had an Endoscopy done. Prior, the doctor informed me what my out of pocket would be which was $500.00. In early April, I received a statement saying I needed to pay $800.00 for the anesthesia (WHICH I WAS NOT AWARE OF AT ALL) This is extremely stressful for me because I’ve been trying to deal with this since the beginning of April. I received 2 statements dated 4/6/2022 and 5/2/22 stating I needed to pay for the anesthesia. I continue to call FDHS to discuss this. (at least 2-4 times a week). I have spoken to ****** ***, ******, *****, *******. and *****. They were nice but didn't solve my issue. I have spoken to some of them a handful of times. Calling them is useless because a supervisor is not calling me back. If I was made aware that I needed to pay for the anesthesia, I would have never gotten the procedure done because it was too much money. I already paid the doctors office around $500.00 for the procedure. I have spoken to many representatives at FDHS Anesthesia LLS. Over the past 3-4 weeks, I was informed at least 4 times that a supervisor would call me back. I still haven't spoken to one. I called Dena at ************ and left her at least 3 messages. I have also called ***** at ************ and left her at least 3 messages. I have also emailed them and sent messages from www.peryourhealth.com and still no response. At this point I don't know what to do. This is my first time making a report on a company. I have great credit but refuse to pay for this. I have the emails that I sent that I can forward to you if you would like to see them. Please advise on what else I can do. Thank you. *** *** ?

      I apologize for the delay. I called my *****-my insurance company. I spoke to ****** and she seems a bit disappointed in this as well. She informed me my deductible was $505. You can reach her if you have any questions. She can be reached at ************ at extension *****. She is in the process of contacting the doctors office as well.

      Here is the information you requested:

      -date of birth is ***** *** ****
      - Appointment dates: January 27, 2022 and February 10, 2022
      - My account number is ************ (From FDHS anesthesia LLC)

      As I mentioned I tried contacting the anesthesiologist  via phone, email and messager on their website. I finally heard back from **** ***** (Vice President) and one of the managers.  I believe her name is ****, but this issue is not solved.

      I have great credit and always pay my bills. I only thought I had to pay $505 for the entire procedure. Now I’m getting bills ( see attached) saying I owe more money -over $1300. I am not sure who I spoke to on my initial date for the pre-procedure appointment but she clearly said I needed to pay $505. She never mentioned anything else and I feel like I was misled because if I knew that it was going to cost ove over $1200 I would’ve never had the procedure completed.  I know I’m rambling on about this but I just wanna make sure my message is very clear. Because I am not paying these bills.
