I am at job training for CDL. I have been trying for a week to get a medical form completed and returned in order to get medical clearance for my Comercial Learners Permit. The form was returned after 3 days incomplete. I had to send it back again and the phone number for the clinic goes to a call center in charge of 18 clinics. They keep telling me they will send a message to the clinic stating this is urgent. I sent this form on Monday January 16th. It is now January 20th. I have fallen behind in my class because of this. The school is going to send me home today if the form is not returned fully complete by noon today. I insisted on talking to someone in the clinic to get this done right now but the manager in the office tells me that i have to wait for the doctor to come in at noon today to complete the form. I am going to lose my career and my house if this form is not received by noon. I have told the call center this but all they do is keep sending messages. They tell me that there is no one in the office I can talk to. I need someone to inform the doctor that I need this completed before noon today. This clinic has proven to be unreliable even when my life depends on it. The consequences of not getting this form completed and returned by noon today is losing my job which causes me to lose my house and everything in it and I will have no income to survive on. I am tired of them treating this like a non-emergency. I have given them a week to complete this. They keep expecting me have more time, which I have run out of at this point. I can't come into the clinic because I am in another state at school for my job. The phone is my only option.
