I visited the ** of ********************************* in ****, Oregon on December 10, 2021. *************************, **** managed the ** visit. Afterwards, both my health insurance company, **** Insurance, and I received a bill for her time (5 - 10 minutes) totaling $890 (account #**********). Moda cannot pay the bill because the diagnosis code provided on the bill needs to specify "right," "left," or "both sides." **** informed me that this is a very minor fix and that they would pay once the diagnosis code was fixed. My portion of the bill would be $10. In the meantime, **** said to not pay the bill (and I have not). Moda ***************** and I made up to 15 calls to the Central Oregon Emergency Physicians' billing department. Each time, they said that they would fix the code. Each month, they continued to send me a "final notice" demanding $890. Additionally, in May of 2022, they sent me to a debt collector. That necessitated more phone calls to the billing department, and **** and I called the debt collector to inform them of the situation. The debt collector assured me that they would put the account on hold and would not report to a credit agency. Central Oregon Emergency Physicians' billing department continued to fail to correct the billing code. In September 2022, I sent a letter requesting that they correct the billing code (since phone calls were ineffective). I copied the billing department, *************************, PA, and *********************************. On October 7, ************************* called me and said that she would fix the diagnostic code. Since then, I have continued to receive "final notice" bills for $890 and have again been referred to a debt collector. I am pleading for help with this case. I have also filed a complaint with the Oregon Attorney General (#******). Central Oregon Emergency Physicians are harassing me for a relatively large amount of money that I don't owe and have not sufficiently responded to innumerable requests to fix a simple mistake.
