      Amount from original billing statement $2321.94, has since been changed to $1406.94 (UNPAID)Account Number# ************** On 8/**/************************* sent a statement to my home attempting to collect for services rendered on 6/27/23. The statement sent to me was an attempt to defraud me of money and misrepresent my insurance coverage in a clear attempt to have me overpay for my procedure. This was verified after checking my explanation of benefits which confirmed Oregon Imaging Centers attempted to unbundle claims, which is medical billing fraud, and that these claims were then represented as valid in a statement sent to my home. I am aware that this was medical billing fraud as I have done insurance billing as part of my job for almost 13 years. Medical billing fraud is illegal. Unbundling claims is a violation of the False Claims Act as established by the ********** of ******** Not only did Oregon Imaging attempt to bill me for fraudulent claims, it misrepresented my insurance adjustments by fabricating a number on the statement, misleading me into thinking that funds were owed for those denied unbundled claims. The adjustments on the statement not only did not match my insurance statements but were also an entirely made up number. I contacted the billing department and was told an updated statement would be sent, without any acknowledgment of the billing fraud that occurred. I filed a grievance with Moda ************************* who have opened an investigation and taken my account of the events as well as all my evidence including scans of said statements. I have also contacted the ***** healthcare fraud reporting service, Oregon ********** of ******* Consumer ************** as well as the Oregon ***************** I would be happy to provide any supporting documentation as Oregon Imaging Center has deleted the original statement from their payment portal. My paper copies and scans are the only evidence that exists.

      August 25,2021 I made a mistake in paying my bills online. I owed Oregon Endoscopy center $300, & mistakenly sent the money to Oregon Imaging without realizing what I had done. In Oct. I receive a **** once again from Oregon Endoscopy. I called to see why & found out they had not received the $300. I wont go into detail, but figured out eventually what I had done & Immediately called Oregon Imaging *************) to tell them what I had done. Talking to **** she found the money & confirmed we did not owe them anything, and would return my money. I immediately called Oregon Endoscopy back & told them what Id done & ********************** was returning the $300 could I wait to pay the **** when the money came back?. They were very kind & assured me it was okpay it when you get it backThis was 11/912/3 I called Oregon Imaging again & spoke with Blonca who searched it all out & found **** had done nothing about the situation at all/very apologetic, would get $300 to me right away! 12/23 called again-spoke to HoulaCheck staged ready to sign, Id have it by the 1st of month! Now ashamed & embarrased to keep telling Oregon Endoscopy (***) the moneys coming,- -wed saved enough & paid them off now they were so kind about it!!1/5 Blonca Now embarrassed herself about situationI told her I would give them till the end of the ******** was going to take action against them, I was done with the lies..she understood & said the check is mailedI called back-I believe the following Mon. 1/10 ..& demanded to talk to whoever was in charge.there.I got Blonca again she put me on hold awhile- came back with name ***** Schaford or Schfert (?) ************ to call. & of course-She didnt answer - left message of demand for $300. Blonca called me back in 2 hrs. Stacys earphones are broke cant call me, but check is in the mail! How did she call you? Oh she emailed me, Blonca said. We still have not received the money! 1/18/22. Can you help us? Please!

         Thank you so dearly for your helpwe did get our money, finally , with your help, Im sure!!!!   We really appreciate your help as we were at a dead end.didnt know where to go.Its nice having a Big Bad Brother -they think -on our side for a change-but, we know your just the Best Big Brother- EVER!
