      I received surgery on July 28th for gallbladder removal surgery and used an anesthesiologist from the Oregon anesthesiologist group. I was told by ************************, where I had my surgery that I could call to prepay for my anesthesiologist to receive a discount. I called Oregon anesthesiologist group on July 27th 2022 to inform them that I would be having surgery the next day, but I did not have insurance and would like to prepay with a credit card over the phone. They took my credit card information and I was charged $1, ******. I inform them that I needed an itemized statement so that I could get reimbursed through a ministry's healthcare sharing program called Samaritan ministries. They said they would not be able to get me an itemized statements until 2 weeks after my surgery. They informed me if I called back in 2 weeks they would be able to get me the information I needed. I called them 2 weeks after my surgery and they said they did not have the codes and would not be able to get me the itemized statement and to call back next week. I called every week to the Oregon anesthesiologist group for a month and a half after my surgery with no help. I continue to call the Oregon anesthesiologist group multiple times per month and after many attempts I was finally able to get a hold of a supervisor named ***** in October 2022. She informed me she was looking into the matter but needed to check to make sure the anesthesiologist was paid. She said she's going to surgery beginning of November and we get back to me until mid November. I've called November and December and continue to leave her messages with no phone call or email back. I I spent over 30 hours with my surgery center, the Oregon anesthesiologist group and trying to get a hold of the anesthesiologist himself to straighten out the matter with no resolution. All I need is an itemized statement of my payment on July 27th showing what I paid in the discount I received. Please help.

      The ambulatory surgery center where the complainant had surgery was a site utilizing OAG-employed and non-OAG anesthesiologists. The complainants procedure was originally scheduled with an OAG anesthesiologist but was subsequently rescheduled with a non-OAG anesthesiologist, Dr. ******************** However, the complainant had prepaid OAG for her anesthesia. In an effort to minimize customer service issues, OAG refunded the complainants prepayment directly to Dr. ******************* for his anesthesia services for the patient's procedure. When the complainant requested a copy of her itemized statement from OAG, we could not provide one to her as she was not an OAG patient but a patient of Dr. ******************** OAG tried multiple times (by phone and email) to obtain an itemized statement from ******************* billing service but encountered issues until we were able to reach Dr. ******************* directly. Dr. ******************* mailed the complainant, his patient, an itemized statement on 12/13/2022.

      I spoke with the complainant directly on 1/4/2023 and confirmed that this matter has been resolved to her satisfaction as she received an itemized statement for her anesthesia services performed by Dr. ******************** Should you have any questions or require additional information, please do not hesitate to contact me.       THEY WERE ABLE TO PUT ME IN CONTACT WITH SOMEONE WHO IS ABLE TO GET ME AN ITEMIZED STATEMENT.
