    - Date of transaction for routine dental services: 11/09/2022. - Amount paid at time of service: $191.00. That day Dr. *******'s office said they were out of network with ***** Dental. They said that any refund issued by ***** Dental would be payable by ***** Dental to me. I received a check from ******* Dental Management LLC dated 2/06/2023 for $43.00. - I filed grievances with ***** Dental on 12/30/22 and 2/03/23. - Nature of the dispute: Per a letter from ***** Dental dated 3/03/23, Dr. ******* was in network on 11/09/22 and my patient responsibility was only $24.00. I am requesting reimbursement of the additional overcharge of $124.00. To date, Dr. *******'s office refuses to discuss the matter, saying they were in network on 11/09/22 and therefore do not owe me any additional reimbursement. - Additional details: I went for a routine dental visit on 11/09/2022. At that visit, I was told that Dr. ******* was not in the ***** Dental network and that I owed $191 that day for services rendered; I paid $191.00. When I phoned ***** Dental to verify the out-of-network status, ***** Dental told me that Dr. ******* was in network. I called my provider to ask about this again and they denied being in network. I finally received a response from my second inquiry with ***** Dental confirming Dr. *******'s status in November. Due to a death in my immediate family, I wasn't able to contact Dr. *******'s office again until today. They are resolve in saying they were not in network with ***** Dental on 11/09/22.

    1) Unprofessional business practice with the secretary speaking for my insurance company with the assurance that my insurance would fully cover the procedure they were recommending. They say they do not speak for insurance companies, but in fact that's exactly what the secretary did. 2) Halfway through pulling my tooth they decided they could not do it and sent me off to another dentist over an hour away which I also had to pay $500 for out of pocket. 3) The first bridge they then ordered for me did not fit and I had to come back two weeks later, take off work again, get numbed all over again and refitted with another bridge. 4) To this day, my entire set of teeth hurt with increased sensitivity, ever since my experience at Smoketown Family Dentistry.
