      On April 14th, I went for preventative care exam which is 100% covered by my insurance. In a few weeks I have received a bill of 192 dollars. I was also charged for my blood work test because this provider submitted wrong billing codes. I called multiple time the office and to the insurance. Insurance is saying wrong codes were sent, provider is not willing to resubmit the correct codes.

      The last week of July I made an appointment for preventative yearly care which is required by my health insurance. I made an appointment for August 5 which I went to Dr D************* office for my preventative care appointment on that date but because he is a scammer and talked to me about my medicine he billed me for a office visit than when I went for a follow up from my blood work 3 weeks later on August 25 he billed me again for an office visit. He is scammer of a doctor and is billing patients incorrectly.

      Good day ****,

      Your letter states this may be publicly posted, so please know it would be a huge HIPAA violation if we addressed this properly as to why patient was in office etc and what was billed.

      Patient should check with insurance company for explanation of coverage and benefits.

      Patient could also have the insurance company question the charges.

      I did have the Associate Director of Revenue Process do a fact pattern of the charges and it was coded/billed properly.

      We also had the Account Receivable Rep dept talk with patient.

      Balance due is appropriate.
