            My child was treated at Revistalist three years ago. We just went back for a repeated treatment on April 4th, 2022. They have no records from her nine previous sessions. They took over $3000 worth of our money for the previous treatments, yet have no records of her at all. I would really like her medical records, so her new provider can use them in her treatment.

            I was saying here in January 2022 I paid them $350 at the time of service. They are a PPO of my insurance and when I got the paperwork what I should’ve paid from my insurance they said I should’ve only paid $115. I have contacted Revivalist several times and no one contact me back. Also my insurance has contacted Revivalist and tried to talk to billing and no one returned my call, my insurance will pay for this type of procedure that I had bought infusion if they would’ve coded it and sent it to my insurance which they did not send the Katamine code to my insurance. My insurance has contacted them for times I have Paul and talk to them also.. I have also went over there to try to get the code numbers. I only wish they would contact me with the codes and correct diagnosis.. My insurance will pay if they will put down the correct diagnoses and code numbers. Please contact me and no one does.. the procedure did help me but I have paid over 2000 and $400 out of my pocket plus my insurance has paid also. I don’t understand Revivalist not trying to help the patient. Thank you.
