      On Saturday, the 2nd of July, 2022 I broke a temporary crown on the front tooth that had been installed by main dental provider the week prior. Because my provider was closed on Saturday and because the tooth was sore I had to go and see the open dentist which happened to be All Heart Dental in *********, **, ********************************. The office agreed to take a look at my tooth. I notified three different employees (front desk, assistants, ******* ******** that I was there only to get a temporary crown.The office however made X-rays and after evaluation offered me services that I had to decline three times. I had to insist that they were to proceed only with installing the new temporary crown. Before doing the temporary the Allheart dental office charged me $135 although my dental coverage is 90% and at that moment I reached the stop loss which made all dental work of that cost free. Since it was an emergency, and I could get reimbursed from my insurance I paid $135 upfront. The assistant made a temporary crown for me and I was discharged. I have never received from them a copy of the itemized receipt and was only given a copy of the register receipt that contained no scope of work. When I submitted that receipt to my insurance company I received a response that they needed an itemized one. When I visited the office on 8/5/2022 the two ladies who work up front were acting incredibly impolite, stating that they won't give me a copy of the itemized receipt whatsoever. They mentioned to me that they would only give it to me when my insurance would pay them. The release of the itemized receipt is my right, it concerns my health and my finances and cannot be contingent upon the payment of the insurance company. I looked up the claim submitted to my insurance company and realized that it was fraudulent: it included images and evaluation but no charge for the crown - the very reason I went there. I was charged for what I did not need and was declined an itemized receipt.
