      I went to this dentist on January 11th, 2022. They performed a procedure that was not covered by my insurance, although the office manager told me during the visit that it would be covered. Since then, the dental office has gone out of business (I think), or been purchased by DentalWorx. That company has harassed me repeatedly by sending me bills over and over again for the last year and a half. I have written and faxed that company each and every time I received a bill from them. They have not responded to any of my attempts to reach them. In good faith, I even paid half of the balance (it was $93.00 and I paid $46.50), and asked them to write off the other half. Instead, they just began sending me bills for the remaining balance. I am tired of writing and faxing this company and tired of being harassed with the nonstop bills.

      I entered into an orthodontic service contract with Parmer Dental on 05/24/2022. Upon entering into the contract, I prepaid $1,000 of my $3,275 contract. I financed the remaining balance over the 12-month contract term. I received an email from Parmer Dental on 09/26/2022 stating that they were closing their office and moving patients to ******* Dental Care in South ****** (45 minutes away on a good traffic day).I refused this change because it was a material change to my contact. It would have added 1.5 hours to my total travel time, and I wouldve lost PTO, etc. Instead, I requested to transfer to my orthodontists private practice because it was only 10 minutes away from my office (a much better alternative for me). Parmer Dental competed 129 days of my 365 day contract (35%). Thus, they are entitled to 35% of the contract price we agreed to since they materially changed the contract, not me. Through October, I paid $1700+. They billed me again on December 8th. Even after the transfer request, Parmer Dental continued to bill me after the requested change. They delayed my transfer for nearly two months and begrudgingly refunded me the extra billed months after I submitted a fraud investigation request to my credit card company. I had to issue a vendor payment block on my card to make sure I didnt continue to get billed again.My orthodontic care only ceased with Parmer Dental because they attempted to materially change my ortho service agreement by changing the location where services are rendered. I paid Parmer Dental nearly $2,000, of which only $1,100 was earned. They have refunded me approximately $378 for Oct. and Dec., but I am still owed $411.30 of the prepaid amounts.They have published the address of their new practice at an incorrect location to make it appear they are located closer to their old location than actual. Their actual location is 45 minutes further away than the one currently being advertised in ****** Maps.

      In January 2022, I enrolled in *********************** dental insurance plan because it covered dental cleanings. I made an appointment with *********** at Parmer Dental, ******, ** , one of the service providers approved by Guardian, for February 1, 2022, the first day of my dental coverage. On that appointment, the office manager, *******, told me that I needed dental X-rays before the cleaning and repeatedly assured me that this is covered by my insurance. After that appointment, I received several bills for $295 worth of X rays. I called several times and kept receiving notices of the unpaid $295. *******, the office manager, assures me that it has to be submitted to the insurance *** Today, May 13, 2022, I read the attached that says they are placing this **** with a licensed collection agency. I called *******. She assured me again stating she has contacted the corporate office to stop with sending this to the collection agency. These are not xrays I ever wanted or asked for, as I was there for a cleaning and nothing else. I only agreed to the xray due to their insistence that they were free to me in any event. I am willing to settle the matter for $100, even though I think that paying anything is unfair since I was told repeatedly that it was a free service.
